A Special Enrollment Period (SEP) exists for individuals whose Medicare Advantage provider network undergoes a transition. This specific SEP is categorized as a "Significant Change in Provider Network". It is intended to provide enrollment flexibility when a major hospital system or a large group of physicians terminates its contract with a Medicare Advantage plan.

Medicare Advantage plans include several plan types designed for different eligibility groups. One type is the Chronic Condition Special Needs Plan, commonly called a C-SNP.

C-SNPs are Medicare Advantage plans for people with certain severe or disabling chronic conditions. Diabetes, chronic heart failure, and cardiovascular conditions are among the most common areas served by these plans.

The number of C-SNP options is changing in many areas, including parts of the Pacific Northwest.

Because plan availability can change each year, it is useful to understand how these plans work, who may qualify, and when enrollment is possible.

Medicare decisions are personal: and where you live can make a difference in the plans and resources available to you.

Working with a local Medicare broker gives you more than a list of options. You receive personal guidance from someone who understands the community, listens to your concerns, and can help explain how Medicare may fit with your doctors, prescriptions, budget, retirement plans, or Medicaid coverage.

A local broker can help you:

Understand your Medicare choices
Compare available plan options
Review premiums, copays, networks, and drug coverage
Prepare for enrollment deadlines
Revisit your coverage when your needs change

Medicare can be confusing, and no single plan is right for everyone. Plan availability, costs, and benefits vary by location and individual circumstances, so take time to review your options carefully.

If you are turning 65, leaving employer coverage, or have both Medicare and Medicaid, Johnson Medicare Solutions is here to provide education and support: at no cost to you. Reach out with your questions. We are happy to help you navigate Medicare with greater confidence.

Medicare Advantage plans frequently outsource the management of “extra” benefits to third-party administrative entities, such as American Specialty Health (ASH). These organizations act as intermediaries between the insurance carrier and the healthcare provider.

Many small clinical offices, including massage therapists and naturopathic doctors, may not participate in these specific third-party networks. This lack of participation is often attributed to high administrative requirements and low reimbursement rates established by the managing vendors. Consequently, a “network desert” is created where the benefit exists on paper but few local providers accept the coverage.
