Pharmacists do a lot more than just fill bottles; we are trained to look at the whole picture of your health. I can’t tell you how many times I’ve caught a dose that was a bit too high or a combination of drugs that just wasn't safe for a patient's specific condition. When we have the time to really consult with you and look at your history, we catch those serious errors before they ever leave the pharmacy. We see quite a few children, and even some adults, who just can't swallow a traditional pill without panicking. When that happens, I usually suggest an oral disintegrating tablet. It’s this tiny, sugar-based tablet that just melts right in your mouth. Because it dissolves so quickly and is absorbed through the lining of the mouth, it often works faster since it bypasses the liver at the start. Erica in my lab is excellent at converting these, and it makes a world of difference for people with throat issues or a strong gag reflex. Roger that!

People often ask if compounding is the same as changing a prescription. In a way, it is, because we are changing the form of the medication, like turning a capsule into a liquid or a dissolvable tablet. Because of that, I’m always in close contact with your doctor. I’ll call them up and explain that a patient is struggling to swallow or needs a different delivery method. Most doctors trust us because they know we understand the chemistry behind the drug, and they're usually happy to approve the change to make sure you're actually getting the treatment you need.

Getting kids to take their medicine shouldn't feel like a battle, but I know it often does. My best advice? Flavoring is your absolute best friend. We have about twenty-five different flavors in the lab, everything from cotton candy to bubblegum, because kids fight a lot less when things don't taste terrible. Another quick trick: try using a syringe instead of a spoon and aim it toward the inside of the cheek. It helps them swallow the dose without having to taste every single drop. Roger that!

I had a patient ask me recently about what actually goes into a capsule besides the medicine itself. Most pharmacies use standard fillers, but here’s the thing, many of those are made from starches like corn or potato. If you have a food allergy, that can be a real problem. The beauty of compounding is that we can adjust every single piece. If you tell me a specific ingredient bothers you, we just leave it out and build your capsule with a filler that works for your body. We think about the parts of the medicine that most people don't even realize are there. Roger that!

In the world of compounding, the "base" is the vehicle that delivers your medicine. We don't believe in a one-size-fits-all approach.

If we are treating a skin infection, we use an ointment that stays on the surface to protect the area. If we are working with hormone replacement therapy, we use specialized bases designed to carry those hormones deep into the tissue where they are needed most. Every base is chosen with a specific purpose to ensure your treatment plan is as effective as possible.

Stop right there! If your medication is labeled "Delayed Release," crushing it can be dangerous. These tablets are specially coated to survive the acidic environment of your stomach so they can activate later in the small intestine.

Crushing them removes that protection, which can lead to stomach irritation or the medication being destroyed before it even works. If you're having trouble taking your tablets whole, let’s talk about compounding a different form that is safe for you.
