Our final inspection was delayed until Thursday at 1 PM, so we’re hoping to resume in-person appointments shortly after. Thank you SO much to everyone who has been flexible with scheduling this week! 💙

We currently share space with West O Glow Luxury Tanning & Wellness inside Prim Beauty Parlour in Downtown Elkhorn, where we offer limited availability for:
• Lab draws
• New patient consultations with APRN Lisa
• Follow-ups for Lisa’s current patients

Even more exciting news...West O Glow Luxury Tanning & Wellness is moving into a larger Elkhorn location soon, and Top Form is moving with them! 🎉 This means more space, more availability, and expanded services, including Botox & IVs!

Sunday 8/30 | 12–4 PM

Lisa will be available for TRT consultations and weight loss consultations, with blood draws potentially available as well (verify at booking).

And the timing might be perfect: our Papio clinic will be closed Friday 8/28 + Saturday 8/29 while we move, so if weekday appointments are tough to fit into your schedule, this is a great chance to snag some rare weekend availability.
