At first glance, most people see wrinkles.

I see something very different.

I see advanced dermatoheliosis, solar elastosis, static and dynamic rhytids, perioral rhytids, crepey texture, cutaneous laxity, and dyschromia from decades of sun exposure and intrinsic aging.

The temptation is often to simply add filler.

Looking back at June 2023, I can hardly believe how much life was packed into a single season.

From the beautiful streets of Lisbon and Porto, to the energy of Tokyo and Kyoto, the vibrant culture of Seoul, and the unforgettable warmth of Bangkok, I was fortunate to travel across the world while attending, presenting, learning, and sharing at international medical congresses. In that single year, I attended more than a dozen conferences, surrounded by some of the brightest minds in medicine and aesthetics.

The photographs bring back memories of long flights, busy lecture halls, meaningful conversations, lifelong friendships, and invaluable lessons. While I may not have the same desire to maintain that pace today, I remain deeply grateful for every opportunity, every mentor, every colleague, and every patient who has been part of this journey.

Looking back, I realize how fortunate I have been—not just for the places I visited, but for the people I met and the knowledge I gained along the way. A heart full of gratitude for the journey, the experiences, and the memories that continue to inspire me every day.

Looking back at June 2023, I can hardly believe how much life was packed into a single season.

Thursday was a lip day. 💋

Some days are filled with lasers. Some days are all about skin rejuvenation. Some days it’s Botox, PRP, acne scars, or facial balancing.

Today was all about lips.

Looking through these photos tonight, I found myself smiling—not because of the filler, but because of the people behind these results.

Some of these patients had never had lip filler before. Some were nervous. Some were convinced they would never do fillers because of stories they had heard or experiences they had elsewhere.

Yet somehow, during the consultation, we build trust.

We talk. We laugh. We discuss their concerns, their goals, and what they hope to see when they look in the mirror.

Most are not asking for bigger lips.

They want softer lines.
Better balance.
A fresher appearance.
They want to look like themselves.

And that’s exactly what I want for them too.

These photos were taken immediately after treatment. No filters. No editing. No waiting weeks for swelling to settle. Just real patients and real results.

Are all the lines gone? No.

Should they be? Not necessarily.

To me, beautiful lips are not about creating a different person. They are about restoring harmony, preserving natural expression, and helping patients feel more confident while still looking completely like themselves.

After more than 30 years in medicine, I still feel honored every time a patient trusts me with their face.

That trust is the greatest privilege of all.

PDO Threads or Surgery?

In this video, I focus on just two points: why PDO threads are unlikely to help this patient and why surgery may be a more appropriate option. However, what you don’t see is the much longer discussion that happens during a consultation.

Skin laxity is not a diagnosis. It is a finding.

The underlying cause may be aging, photoaging, chronic sun exposure, inflammation (inflammaging), collagen loss, significant weight loss, GLP-1–associated weight loss, genetics, connective tissue disorders such as Ehlers-Danlos syndrome, hormonal changes, smoking, illness, or a combination of many factors.

This is where patients can get into trouble. Many people simply pull on their skin, like what I am demonstrating in this video, and assume a thread lift is the answer. Non-surgical procedures can be very appealing because they appear simple, quick, and less invasive than surgery.

But treatment should never be selected based on what appears attractive. It should be selected based on understanding the cause of the problem.

Depending on the patient, treatment options may include observation, skincare, weight stabilization, energy-based devices, non-ablative lasers, ablative laser resurfacing, laser coring, dermal fillers, biostimulators, PDO threads, surgery, or a combination approach.

In this particular patient, the discussion is even more complex because she has Ehlers-Danlos syndrome, a connective tissue disorder associated with significant skin laxity. Even surgery can be challenging in these patients. The literature regarding ablative lasers is limited, outcomes may be unpredictable, and realistic expectations are critical.

The goal of a consultation is not to choose a procedure. It is to understand the cause of the problem, review the available options, and determine what is most likely to help—and what is unlikely to help.

Sometimes the most valuable advice is learning what not to do.

Platelet-Rich Plasma (PRP), also known as the “Vampire Facial” or “PRP Facial,” is one of the most popular regenerative treatments in my practice, especially during the summer months.

The patient shown here was looking for a natural approach. She was not interested in dermal fillers and, because it was summer, was not interested in a more aggressive laser procedure.

PRP was an excellent option.

Pocketful of Sunshine ☀️

Twelve years ago, if you had told me that my life was about to take a dramatic turn, I would have laughed and hurried back to the operating room.

At the time, there were no lasers, no injectables, no aesthetic procedures in my world—only medicine, family, and an endless race against the clock.

My life revolved around operating rooms, transplant and vascular surgeries, raising three children, and balancing two demanding medical careers. My husband and I were both physicians, and like many healthcare professionals, work-life balance was more of a dream than a reality.

Most people never see what happens behind the scenes in medicine. Long days. Very early mornings. Being in the operating room by 4 a.m. to prepare for major cases was simply part of life. The work was intense—caring for patients undergoing complex vascular, cardiovascular, thoracic, neurovascular, orthopedic, spine, and pediatric surgeries that often lasted for many hours. The responsibility was enormous, and every day demanded focus, precision, and dedication.

What many people also do not know is that I was not just practicing anesthesia in America. Before coming to this country, I was already a fully board-certified, double board-certified anesthesiologist in India. That experience shaped who I am as a physician and as a person, and it is an important part of my story.

It was a privilege to work alongside extraordinary surgeons, nurses, anesthesiologists, and mentors at Harvard-affiliated hospitals and Yale New Haven Hospital.

Looking back at this video, I see a 48-year-old physician who had no idea she was about to start an entirely new chapter.

Twelve years later, the setting has changed, but the responsibility has not.

I remain grateful for every mentor, every colleague, every patient, and every opportunity that helped shape this journey.

Life has taken me down a path I never expected, and I would choose it all over again.
