US Air Force Medical Service: Synensys has partnered with the Air Force to provide Patient Safety Specialist programming for 10+ years resulting in a 70% reduction in preventable patient harm events. Many of the 70+ medical treatment facilities have reduced serious safety events to zero as a result of the full implementation of system safety practices and high reliability principles. US Air Force Air Mobility Command: Air Mobility Command has been able to develop a customized Line Operations Safety Audit (LOSA) program capable of interfacing and integrating with multiple safety management data sets leading to comprehensive system improvement in safety culture, safety reporting, and threat and error management performance. WellSpan Health: WellSpan developed a comprehensive system safety program for their largest hospital perioperative service line. Calling it “human error” might be the biggest risk of all.
Because most decisions are not made in ideal conditions.

They are made under:
• Time pressure
• Competing priorities
• Incomplete information
• Ambiguous ownership

What appears to be a poor decision is often the result of a system that did not fully support the person making it.
This is where human factors become critical, not as a soft concept, but as an essential part of safety, performance and operational resilience.

If organizations want to prevent harm, they must design environments where the right decision is the easiest one to make.

As part of National Safety Month, Synensys is highlighting how thoughtful system design can reduce risk before failure occurs.

In one anonymized operational setting, personnel were required to make time-sensitive decisions across complex workflows and multiple information sources. The teams were highly capable and deeply experienced, but the system itself introduced unnecessary cognitive load, friction and opportunities for miscommunication.

That is where risk begins to grow.

Our approach: We applied human factors principles to improve performance under real-world conditions by:
• Streamlining workflows to reduce complexity and rework
• Improving how critical information was displayed, prioritized and accessed
• Identifying handoff points where delays or miscommunication could occur
• Reinforcing clear decision cues designed to hold up under stress

Most organizations don’t have a safety problem.
They have a hidden risk problem.

It shows up in places that are easy to overlook:
– Handoffs between teams
– Interfaces between systems
– Ownership that isn’t clearly defined

These are the conditions where risk accumulates—and where incidents often begin.

This June, Synensys joins the National Safety Council in recognizing the 30th anniversary of National Safety Month, an initiative dedicated to preventing injuries and saving lives before they happen.

Throughout the month, we’ll be sharing insights on how organizations can identify and address hidden risk—before it turns into harm.

In complex operational environments, risk is not always obvious. Some of the most significant vulnerabilities emerge quietly across processes, teams and technologies that appear to be functioning normally.

In one anonymized high-risk setting, critical operations were performing “as designed.” Yet beneath the surface, hidden interactions across workflows, responsibilities and decision points were creating conditions where failure could occur with little warning. Traditional assessments did not identify the issue because the system appeared stable until conditions aligned in unexpected ways.

Our approach: System-Level Safety Analysis
To uncover risks not visible through routine reviews, we conducted a system-level analysis focused on:
• Mapping real-world workflows and cross-functional dependencies
• Identifying vulnerabilities at interfaces, handoffs and decision points
• Evaluating how multiple conditions could combine to create unsafe outcomes
• Translating findings into actionable mitigations and operational constraints leaders could implement and sustain

The impact
• Improved visibility into previously unidentified hazards
• Prioritized mitigations based on severity and operational impact
• Increased confidence in leadership oversight and decision-making
• Stronger alignment between operational reality and safety strategy

Medical Laboratory Professionals Week | April 19-25, 2026

Each year, Medical Laboratory Professionals Week highlights the vital role laboratorians play in patient care. This year’s theme, “To Infinity and Beyond for Our Patients,” recognizes the expertise and dedication that support nearly every step of the healthcare journey.

The data you generate, validate, and manage informs clinical decisions, supports research, and moves through complex healthcare systems every day. While much of this work happens behind the scenes, its impact reaches patients across the entire care continuum.
