I'm proud to say that Paragon Pain Solutions has been awarded the GOLD MEDAL in the Pain Management Clinic category for the Philadelphia Inquirer's 2026 Philly Favorites issue.

What makes this especially meaningful is that Paragon is a solo practice. To be recognized alongside (and ultimately ahead of) some of the largest clinics in the region is both humbling and deeply rewarding.

This goes to show just how far persistence, attention to detail, and commitment to quality care can take you.

🔎 CASE STUDY 🔍

It's become really popular to bash the concept of myofascial chains in clinical reasoning recently. And while this can be used by mediocre practitioners to create a narrative that has no basis in reality, this doesn't mean that there is never value in using the concept of myofascial chains for sound clinical reasoning and problem solving.

🤔 Case in point - a client came in today complaining of chronic pain and tightness in her upper hamstrings. She told me that stretching this area in various ways only gave her very temporary relief, and that her symptoms would always return.

🫱 Palpating around revealed that her medial hamstrings were very tight all the way through, and particularly tender at the attachment point. Upon palpating the bottom of her foot, I found a very tight/tender spot where the adductor of her big toe attached.

💪 Muscle testing showed that her medial hamstrings testing super strong, but her lateral hamstring and glutes tested weak. The extensors and flexors of her big toe tested weak as well.

💆🏼‍♀️ After spending a minute or two releasing the big toe adductor, I retested the big toe extensors and flexors and they tested strong! The lateral hamstring and glutes tested strong now as well. And best of all, the tone of her medial hamstrings had now normalized. The tenderness was already 90% better, and I didnt even treat them!

It's true that sometimes we unnecessarily overcomplicate things, but sometimes it really pays to understand how distant parts can affect the function of the whole.

🤔 One of my regular clients came in complaining of pain in her L hip when she performs lunges on her L side during her workouts. She said the pain wrapped around her glutes toward the crease of her hip when she loaded her L leg. Seemed like a problem with the glutes and/or deep hip rotators to me, so I started by investigating there.

💪 Sure enough, muscle testing revealed that her distal L adductor magnus was inhibiting her L glutes and piriformis.

🧐 Long ago we had found that her R leg was 8mm shorter than her left. Overactivity in the adductor group on the long leg side is a very common compensatory pattern for this sort of discrepancy.

💆🏼‍♀️ I released the adductor and activated the glutes and piriformis. When she got up and lunged again her pain was gone!

✅️ Her homework was to release the adductor and activate the glute medius with a lateral leg raise. I also gave her some tips and strategies she could use to help minimize the impact of the short leg during her workouts.
