📢 IMPORTANT: Medicare & Medicaid Changes Are Coming in 2027

If you or someone you care for has both Medicare and Medicaid, changes beginning in January 2027 may affect how coverage, benefits, and services are coordinated.

At Patient Care Coordination (PCC), we know that changes in health coverage can raise questions — especially when you rely on services and supports to remain healthy, safe, and independent.

✔️ Keep your contact information current
✔️ Watch for official notices from Medicare, Medicaid, and your health plan
✔️ Read enrollment and coverage materials carefully
✔️ Don’t ignore letters about your 2027 coverage
✔️ Contact your Service Coordinator if you have questions about how a change may affect your services

🇺🇸 25 Years Later. We Remember. We Honor. We Stand Together.

Today marks 25 years since September 11, 2001—a day that forever changed our nation and the lives of so many families and communities.

We remember the nearly 3,000 lives lost. We honor the first responders, healthcare professionals, service members, volunteers, and ordinary people who demonstrated extraordinary courage. And we recognize the families who have carried the weight of that loss for the past 25 years.

Amid unimaginable tragedy, we also witnessed something powerful: people caring for one another. Neighbors helping neighbors. Strangers becoming sources of strength. Communities standing together.

At Patient Care Coordination, that spirit of compassion, dignity, service, and community continues to resonate deeply with who we are.

May we honor those we lost not only by remembering their lives, but by continuing to build a world defined by kindness, unity, and care for one another.

Smoke from wildfires burning in Canada is affecting air quality across parts of Pennsylvania. Poor air quality can be especially harmful for older adults, children, and individuals living with asthma, COPD, heart disease, or other chronic conditions.

Protect yourself and those you care for:

Stay indoors when air quality is unhealthy, keep windows and doors closed, use a HEPA air purifier when available, limit strenuous outdoor activity, and wear a well-fitted N95 mask when going outside is necessary. Check local conditions regularly through AirNow.gov and follow Pennsylvania public health updates.

Patient Care Coordination Inc. is actively monitoring conditions and remains committed to participant safety, continuity of care, timely communication, and assistance during environmental health events. Participants and caregivers who need support or notice changes in their health should contact their care team. Seek immediate medical attention for severe difficulty breathing, chest pain, confusion, or other serious symptoms.
