Explore the Magic of Motion is launching a pilot program from March 1–April 30, 2026 to strengthen collaboration between Community Health Workers (CHWs), healthcare providers, and community members—improving support for people managing health at home.
💡 Why Join?
This pilot helps refine community-centered support:
Community members: Access education, follow-up, and navigation support to manage health at home.
CHWs: Increase visibility, collaborate with providers, and shape recognition of your expertise.
Healthcare providers: Connect with CHWs to support shared patients, reduce burnout, and improve care continuity.
⏱ Commitment: ~10 hours over 2 months

Nancy is a Licensed Clinical Social Worker with nearly two decades of experience supporting individuals on their healing journeys. Her compassionate, culturally sensitive approach helps people build confidence, resilience, and balance in life — especially through transitions and personal growth.
