Crossroads Hospice & Palliative Care We are a national leader of hospice and palliative care dedicated to helping patients and families find comfort for chronic and life-limiting illnesses.

Since opening our doors in 1995, we have had one philosophy—expect more. Since then we’ve been busy reinventing hospice and palliative care by finding ways to spend more time with patients and families; and developing ideas to help them experience a better quality of life. No matter how you look at it, Crossroads will be there when you need us most. Our patient-to-staff ratio is among the best in

hospice. We provide more daily visits to our patients than anyone in hospice care and will be by your side during the most critical times. We also offer education and guidance to help you make informed decisions about your care. We can assist with advanced care planning, emotional and spiritual support, coordination of care and access to community services.

Five-year-old Masanjay and her family spent a memorable day at the Philadelphia Zoo alongside social worker Pauline Yeanay and RN case manager Maroline Walaka.

The family enjoyed feeding giraffes, sharing lunch together, exploring the zoo, and helping Masanjay choose a special stuffed animal to take home. Most importantly, they created joyful memories together.
