Ponca Circles of Care is conducting a Post-Community Needs Assessment Survey as we come to the end of our grant year. The survey will help us better understand the experiences and needs of the Ponca community in several important areas, including culture and cultural connection, Tribal services and resources, Substance use and Mental health and wellness.
The survey is open to anyone 18 years of age or older who is part of the Ponca community.
As a thank you for your time and participation, individuals who complete the survey will receive a $20 Little Dance gift card.
Your feedback is valuable and will help inform future community efforts and services. We encourage eligible community members to take the time to share their experiences and perspectives.

Your experiences with accessing services, the challenges you've faced, and your ideas for improving support for children, youth, and families are valuable. Your voice can help shape a stronger, more responsive system of care for our community.
