FPCC Diabetes Prevention & Heart Health Program Our next cohort is starting the 2nd week of October 2026, to participate in the 1-yr FPCC Diabetes Prevention Program! Free to participate. What is pre-diabetes?

Link below to share your contact info, and we'll reach out to you to share more info and talk about enrollment. The FPCC Diabetes Prevention & Heart Health Program is a 12-month program with weekly (1st 6 months) and monthly (2nd 6 months) sessions. Weekly sessions focus on lifestyle change strategies to improve nutrition and exercise habits, relying on support from classmates and guidance from a

certified Lifestyle Coach. Monthly sessions are designed to guide the nutrition, exercise, and weight loss goals met during the program. Every class is facilitated by a trained lifestyle coach who will support you in your journey through lessons and discussions, to help you stay motivated to lose weight, gain confidence, establish healthier eating behaviors and identify enjoyable physical activity. Program is for adults aged 18+ who have prediabetes. It is the point in A1C and blood sugar levels that is higher than normal, but not quite high enough to be considered full-blown type-2 diabetes. Take the Pre-diabetes Risk Test today: https://www.cdc.gov/prediabetes/takethetest/

PROGRAM GOALS:
- 4-7% weight loss
- Track food intake
- Work toward accomplishing 150 minutes per week of physical activity
