We incorporate a small team approach in providing compassionate personalized care for each patient. Our primary focus is our patients and their families, who we serve with dignity and respect. We believe that our patients have a right to participate in all decisions about their care. We offer suggestions based on sound medical practices but it is our patient’s life and ultimately our patient’s choice as to what will help them on this very personal journey.

Grief does not keep a schedule. It can arrive in the middle of a grocery store, on an ordinary Tuesday, or in the quiet after everyone else has moved on.

Not every kind of support we offer is medical. Our Transitions program offers free, non-medical support for people facing a serious illness. That can look like help understanding options, connecting to community resources, or simply having someone steady to talk to during an uncertain time. Someone can even participate in our Transitions program and our In-Home Primary Care Program at the same time, receiving both practical guidance and medical care at home. Whatever path you are on, there is something here for you.

Did you know that according to federal Care Compare data, ClearPath completed all seven recommended care processes at the start of care for 99.2 percent of our hospice patients?

In plain terms, that means asking about each person's beliefs, values, and treatment wishes, checking carefully for pain and breathing trouble, and acting on what we find, right from the very first visit.

This is the thorough kind of care we would want for our own families, and it's our honor to help people at the end of their lives.

When ClearPath designed our In-Home Primary Care program, it originally included a monthly membership fee to help cover the cost of traveling to patients.

Before we ever saw our first patient, our leadership made a different decision and eliminated those fees entirely.

Today the program is fully billable to Medicare, Medicaid, and private insurance, with no membership or enrollment fees of any kind.

Some of the most meaningful work at ClearPath is done by people who simply choose to show up. Our hospice volunteers sit with patients, arrange flowers, sew bears, share a conversation, offer families a moment to rest, and bring comfort in quiet, ordinary ways that matter more than they might realize.

There is a moment that catches every dementia caregiver off guard. The first time your loved one looks at you and doesn't know who you are.

It is heartbreaking. It is also the disease, not them.

Dementia care expert Teepa Snow teaches a different response than correction. Validate their reality. Step into their world. Acknowledge what they are saying with simple words like "oh" or "huh." Repeat their phrases back so they know you are listening.

Then become a detective. What is the underlying emotional need? Sometimes a gentle subject change settles it. Sometimes a quiet physical connection, like the hand-under-hand technique, says what words cannot.

This approach reduces distress for both of you and protects the connection, even as the disease evolves.

Getting to a doctor's appointment is not simple for everyone. For seniors managing complex health needs and for the family members caring for them, leaving the house can be the hardest part of the day. That is exactly why our In-Home Primary Care team comes to you. Taylor Garr-Jones, FNP-C, and Helan, our Primary Care LPN, travel across Deschutes, Crook, and Jefferson counties to provide unhurried visits right where you are most comfortable, with plenty of time built into the appointment to talk through what is going on.

There are no membership or enrollment fees; each appointment is billable to Medicare, Medicaid, and private insurance.
