ClearPath Healthcare

ClearPath Healthcare While our name has changed, our dedicated team, our commitment to 24/7 care, and our deep roots in Central Oregon remain the same.

Offering hospice care, transitions, veterans support, bereavement, and now in-home primary care throughout Central Oregon. Hospice care is designed to focus on quality of life for the terminally ill patient. Our goal is to enable patients to be as comfortable and as free of symptoms and pain as possible, so that they live each day as fully as possible. We believe in treating the patient holistical

ly, honoring their life and their wishes. Our philosophy is to provide support for the patient’s emotional, social, and spiritual needs as well as medical symptoms management as part of treating the whole person. We incorporate a small team approach in providing compassionate personalized care for each patient. Our primary focus is our patients and their families, who we serve with dignity and respect. We believe that our patients have a right to participate in all decisions about their care. We offer suggestions based on sound medical practices but it is our patient’s life and ultimately our patient’s choice as to what will help them on this very personal journey.

For someone living with dementia or limited mobility, home should feel like the safest place possible. But small details in any home can have a big impact on health.

Our in-home provider takes the time to notice the things a clinic visit cannot.

Is nutrition supporting your health goals?
Is the lighting safe for nighttime navigation?
Are medications organized and accessible?

Knowing your daily routine in person allows us to give advice that keeps you independent and healthy for longer.

ClearPath In-Home Primary Care is now accepting patients. No membership fee. 100% insurance billable.

Today, we remember those who gave everything for our country.

Their bravery will not be forgotten. Their families carry their absence every day, and we hold space for that loss alongside them.

ClearPath Healthcare is humbled to walk alongside the veterans of Central Oregon and the families of those we have lost.

Wishing everyone a peaceful Memorial Day.

Your home environment tells a story your medical chart cannot.

When ClearPath brings primary care to your door, we aren't just checking vitals. We see how you move through your kitchen, what is on your shelves, and where the rugs sit in the hallway. Those details matter. They reveal health risks, nutritional gaps, and safety concerns that a clinic visit can miss.

It is personalized medicine built around the way you actually live. No membership fee. 100% insurance billable.

For three days each June, ClearPath Healthcare brings Central Oregon youth together to enjoy a summer camp that goes beyond games and laughter. It is an opportunity for campers to unite and discover they are not alone in remembering the people they love that are no longer with us. They show up because it combines art, learning, and fun, and its a place where grief is allowed. A place where it's okay to grieve. Camp Sunrise happens because of the volunteers and counselors who stay present as quiet listeners. People who understand and can sit at a craft table and be patient with a hard conversation. It's a special weekend.

Camp Sunrise relies on volunteers. If you have a heart for kids and a few free days in June, Camp Sunrise has a place for you.

Camp dates: June 19-21 (overnight, ages 7-12) and June 20 (teen retreat, ages 13-17)

Grief doesn't move in a straight line.

Some days, you are overcome with grief in a public place. Some days those feelings are quiet. For seniors carrying grief alongside health changes and life transitions, the weight can feel especially heavy.

Our new blog explores how grief and stress are connected, what actually helps, and how moving forward can include remembering. Kat Crawford, our Bereavement Coordinator and Camp Sunrise Director, shares simple steps that can make a real difference.

Healing is possible. You are not alone.
