Yesterday, the teams from Touching Hearts at Home and Together In Caring had the wonderful privilege of treating the amazing social work team at The Friendly Home to a delicious lunch! 🥪🎉

While connecting, we shared how our two organizations partner closely to provide seamless care and smooth discharges home. Whether a resident is in skilled nursing or finishing up short-term rehab, our combined team approach ensures they transition back home safely, comfortably, and with total peace of mind.

Please join us in welcoming Melina Bain to the Together in Caring team as our newest Care Manager Assistant in the Albany office!

Melina is currently pursuing a degree in Health Sciences at Siena University and plans to continue her education in Physical Therapy at Russell Sage College. Inspired by her experiences volunteering in care homes, she is passionate about supporting older adults and making a positive impact through compassion, connection, and care.

We are excited to have Melina join our team and look forward to the kindness, dedication, and fresh perspective she will bring to the individuals and families we serve throughout the Capital Region!

Meet Chelsea đź‘‹ and help us congratulate her on her third work anniversary today!

Chelsea’s passion for working with older adults began at just 15 years old, and it’s been at the center of her career ever since. From serving as a civilian nurse in the U.S. Army in Hawaii to leadership roles in senior living communities, she brings a depth of experience across admissions, case management, and clinical leadership.

As a Licensed Practical Nurse, Certified Care Manager, Clinical Supervisor, and PAC Certified Trainer, Chelsea combines clinical expertise with a compassionate, person-centered approach to care. She is also trained in Teepa Snow’s Positive Approach® to Care, bringing specialized knowledge in dementia support to the families we serve.

Chelsea often shares that working with older adults feels like “home”.

Outside of work, she enjoys spending time with her daughter Gianna, along with boating, fishing, grilling, and being with family and friends.
