PDSA Cycle Project Funding

The Chronic Disease Prevention Program (CDPP) is awarding subgrant funds to support projects for the Wyoming Cardiovascular Disease Coalition. The Coalition’s purpose is to improve cardiovascular health outcomes for all persons, particularly those with poor cardiovascular health or at high risk, through direct interventions at the clinical and community levels.

Each project will use Plan-Do-Study-Act (PDSA) cycles of improvement and collect data throughout the project period. Applicants will be able to craft a project proposal based on six project options developed by the Wyoming Cardiovascular Disease Coalition, aimed at improving cardiovascular disease outcomes.
