The Payer Said No Twice. Here's What I Tell Providers to Do Next.
When a provider calls me frustrated — they appealed, the payer recycled the same denial language, and now they're wondering if it's even worth fighting — my answer is always the same.

You've been playing on the payer's field. Let's change the field.

Most out-of-network providers default to 2 paths when a claim gets underpaid: appeal it, or negotiate informally and hope the payer budges. Both have their place. But here's the problem — both keep the payer in control of the outcome.

Appeals? The same organization that underpaid you decides how seriously to review your dispute. Informal settlements? They depend on leverage that shifts case to case, contact to contact, mood to mood. As many of you know, I have been working with Dr. Proodian for a while. He is putting out some great articles on LinkedIn, if you have a chance to connect with him there or Follow his company, Proodian Healthcare. Here is one of his recent articles.

In thirty-two years of clinical practice, I have sat across from more than 250,000 patient visits — one on one, no audience, no agenda beyond one thing, understand what is happening inside this person well enough to help them.

That number is not a credential. It is a classroom.

And the single most important lesson that classroom has taught me is this: people do not resist help. They resist not feeling heard. The moment someone feels truly understood — not agreed with, understood — their defenses come down. Something in the body relaxes. The conversation changes.

That is emotional rapport. And it is disappearing from our culture at an alarming rate.

We have confused having an opinion with having a conversation. We walk into rooms — boardrooms, dinner tables, comment sections — already knowing what we want to say, already positioned to win. What we have lost is the instinct to enter a conversation to learn something. To find the ground beneath our differences before we plant a flag on either side of them.

I was a middle school teacher before I became a clinician. A year in a classroom with twelve and thirteen year olds teaches you something most leadership programs won't: if you cannot connect with the person in front of you first, nothing you know matters. The information you carry is irrelevant until the relationship earns it a seat at the table.

That principle has guided every keynote I have delivered. Every radio broadcast I hosted. Every new patient who walked through the door not sure if they trusted me yet. My job in those first moments was never to impress. It was to make one thing clear: I am here to understand you, not to sell you something.

There is an enormous difference between those two postures, and people feel it immediately.

The most practical thing I can offer anyone navigating our current cultural climate — in business, in leadership, in healthcare, in their personal lives — is this: find the non-debatables first.

Every person you will ever meet wants to feel well. Wants their family to be safe. Wants to be seen as capable and good. Wants their work to mean something. These are not political positions. These are not negotiable. These are the human foundation that sits under every disagreement waiting to erupt on the surface.

When you begin a conversation there — at the foundation — something shifts. The other person is no longer a position to dismantle. They become a person to understand. And people who feel understood do not need to be right. They become willing to think.

This is not soft leadership. This is the highest form of it.

Thirty-two years of sitting alone in a room with someone at their most vulnerable has convinced me of one thing: connection is not a soft skill. It is the primary skill. Everything else — clinical knowledge, business acumen, technical expertise — is secondary to the ability to make another human being feel genuinely seen before you offer them a single answer.

We are not short on information. We are short on the willingness to listen before we speak.

That is the real epidemic. And it has a cure.

It starts with you deciding, before the next conversation you walk into, that your first job is not to be understood. It is to understand.

A realtor friend of mine, not an attorney, was sitting in the middle of a nine-month legal battle, four parties fighting, and an opposing attorney with 40 years of litigation experience trying to have him removed from the case entirely.
