PACE of Southwest Michigan

PACE of Southwest Michigan The Program of All-Inclusive Care for the Elderly (PACE) helps older adults remain at home by providing integrated medical and social services.

PACE (Program of All-inclusive Care for the Elderly) is a unique program of long term care that allows elderly adults to remain in their home. We work with you and your loved ones to provide the support at home, medical treatment, and social interaction needed to remain strong and independent. The key to your success is our care team. This is a team of physicians, nurses, pharmacists, social worke

rs, therapists, and transportation staff who are familiar with you and your needs. The care team meets daily to discuss all aspects of your care and provide services when appropriate. Our physician is a geriatrician specializing in the care of adults as they age. We encourage caregivers to remain involved by providing frequent communication and supportive services including respite care. PACE of Southwest Michigan serves Berrien, and parts of Cass and Van Buren Counties. Note: A PACE participant may be fully liable for the cost of medical services from an out-of-network provider or without previous authorization (except for emergency services).

PACE of Southwest Michigan had the wonderful opportunity to sit down with Pat Moody as he went behind the scenes to learn more about how the Program of All-Inclusive Care for the Elderly (PACE) works.

Pat was joined by Paula Shimulunas, Director of Clinical Operations, and Valerie Ryder, a family caregiver who has experienced firsthand the difference PACE can make.

Paula shared how our interdisciplinary team works together to provide comprehensive care and support, including our full-service clinic and primary care, transportation, home care, day center, pharmacy, therapy, spiritual care, and so much more. She also explained who may qualify for PACE and how easy the enrollment process can be.

Valerie shared her personal experience of walking into her first care meeting as the new guardian of her brother and his wife. What started as an overwhelming experience quickly became one of relief, knowing that she had an entire team of professionals standing beside her and helping support her family.

πŸ’™ That is what PACE is all about β€” supporting our participants, their families, and caregivers while helping older adults remain independent and safely living in their own homes.

If someone in your life is 55 or older, lives in Berrien County or parts of Cass or Van Buren County, and could benefit from the comprehensive services PACE provides, we would love to connect with you!

We've been busy, to say the least, but what we experienced last Thursday was truly fulfilling. It was uplifting, comforting, and a reminder of why we do what we do. It filled our hearts and our cups.

Last Thursday, alongside our community members, Board members, Chamber members, the wonderful Niles community, and our dedicated team, we celebrated the Ribbon Cutting Ceremony for our new Niles location.

This milestone didn't happen overnight. It came after years of hard work, challenges, and perseverance. Through every trial and tribulation, our team remained resilientβ€”and together, we made it happen.

It is an incredible honor to expand our services in the Niles area and have the opportunity to care for up to 150 seniors in Berrien and Cass counties, helping them remain safely and independently in the homes they love.

Thank you to everyone who attended our celebration and to everyone who played a part in making this milestone possible. Your support means the world to us, and we are excited to begin this next chapter of serving our community.

To learn more about PACE of Southwest Michigan and the services we provide, call 269-408-4350 to speak with a member of our Enrollment Department. We're here to answer your questions and help you determine if PACE is the right fit for you or your loved one.
