We've had several prospective clients ask us this lately.

โ€œAre you based in the U.S.?โ€
Yes. We are.

What's interesting is that people seem genuinely happy when we say yes.
Most don't really explain why they're asking, and I don't want to assume.
But it has made me realize that practices really care about knowing who is handling their billing.

With us, you're not sending your billing off somewhere and wondering who has it.

My husband and I and our small team actually do the billing.

We know our clients. We know their staff. They know us. And if something comes up, they can actually talk to us.

I think there's a lot of value in that.

If I have to fix the same billing problem over and over again, eventually I'm going to start asking why.

Not why the claim has a problem.

Why do we keep having this problem in the first place?

Sometimes something isn't being done correctly in the office. Sometimes we're missing information. Sometimes there's a misunderstanding between us and the staff.

Whatever it is, I'd rather figure it out.

I don't want to spend the next six months correcting the same thing every week if one conversation can solve it.

That's something we've learned to do more and more as billers.

Don't just fix what's in front of you.

Figure out why it keeps landing in front of you.

This is something I think practices really need to understand.

Getting paid doesn't automatically mean your billing is being done correctly.

There are ways to push claims through that may get them paid today but aren't necessarily the right way to bill them. Overusing modifiers is one example.

And that's what makes this tricky. If money is coming in, everything can look like it's working.

I would much rather take the time to research something we're unsure about and know why we're billing it the way we are.

Because getting the payment isn't the only goal.

I want our clients to be able to keep the payment too.

One of the biggest things I've learned in medical billing:

The insurance card does not always tell the whole story.

I wish it did.

Sometimes the company whose name is on the card isn't ultimately where the claim needs to go.

There can be different programs, administrators and networks underneath one recognizable insurance company. Sometimes you have to follow quite a trail to figure out who you're actually dealing with.

This is one reason I'm not a fan of just tossing a claim somewhere and seeing what happens.

Figure out what coverage the patient actually has. Find the right source. Verify the information. Then bill it.

It can take more work in the beginning, but it saves a whole lot of unnecessary work later.

We have a client who will absolutely ask us to chase down 30 cents.

And yes, we chase down the 30 cents.

When we first started working with him, he wasn't exactly convinced we were going to be any better than the billers he'd had before.

He had plenty to grumble about.

Then he received his first insurance check after we'd taken over. It was around $700 after going months without anyone billing for him.

He still keeps us on our toes.

Over time we've figured out a really good way to communicate with each other. He knows we're going to respond and take his concerns seriously, and we've figured out a system that works well for everyone.

Even when it's 30 cents.

Every client is different. Part of being a good billing company is learning how to work well with the actual people behind the practice.

Sometimes when we take over billing, I expect to find a complicated problem.

And then I look at what has actually been happening and think, how was all of this just left sitting here?

We've taken over more than a year of backlog before. About half of it was already past timely filing.

No complicated insurance issue caused that. The work simply wasn't getting done.

Medical billing can absolutely get complicated. But sometimes the biggest problems aren't complicated at all.

Claims need to be submitted. Unpaid claims need to be followed up on. Problems need to be resolved. And someone needs to notice when something isn't getting done.

Small practice or large practice, your billing deserves consistent attention.

Sometimes good billing really does come down to doing the work.

The billing process that worked when a practice was smaller may not work as well as volume increases.

More patients and more appointments can also mean more follow-up, more outstanding revenue to manage, and more opportunities for things to fall behind.

Sometimes growth exposes weaknesses that were easy to overlook when the workload was smaller.

Your practice may have changed significantly over time. It is worth asking whether the processes supporting it have changed too.

Has your billing operation grown along with your practice?

A high clean claim rate is a good thing, but it is only one measure of how well your revenue cycle is working.

A claim can be accepted and processed without ultimately producing the payment you expected.

That is why looking at one positive number can sometimes give an incomplete picture.

The real question is whether the entire process is producing the results your practice needs.

Getting a claim submitted is important, but there is still plenty that can happen before payment is received.

Claims may need follow-up, additional information may be requested, or something may need to be resolved before the process is complete.

That is why follow-through matters just as much as getting claims out the door.

A strong billing process keeps track of what happens after submission and makes sure unfinished business does not get forgotten.

Not every billing problem is easy to spot.

Claims may be getting submitted and payments may be coming in, while problems are quietly building underneath the surface.

Recurring denials, delayed follow-up, aging claims, or other patterns can be easy to miss when the focus is simply on getting the daily billing work done.

Taking a broader look at your billing can reveal issues that individual claims may not show.
