A 2023 systematic review analyzing 15 studies found that prone positioning in trauma and surgical patients:

🚑 Trauma Doesn't End in the ER

Severe chest trauma can trigger a cascade of inflammation that leads to Acute Respiratory Distress Syndrome (ARDS), even after initial stabilization.

Rib fractures, pulmonary contusions, and blunt thoracic injuries can impair oxygen exchange and cause progressive respiratory failure.

For patients who develop moderate-to-severe ARDS, prone positioning has been shown to improve oxygenation by redistributing ventilation, reducing ventilator-induced lung injury, and improving lung recruitment.

Recent evidence suggests trauma patients may experience the same oxygenation and mortality benefits from prone therapy as other ARDS populations, with no significant increase in major complications.

☀️Summer is trauma season.

🌡️As temperatures rise, so does trauma volume. Motor vehicle collisions, motorcycle accidents, recreational injuries, drownings, and other high-acuity events increase significantly during the summer months, placing added strain on emergency departments, trauma centers, and ICUs.

Many of these patients develop acute respiratory failure or Acute Respiratory Distress Syndrome (ARDS) as a result of chest trauma, pulmonary contusions, massive transfusions, sepsis, or systemic inflammatory responses to injury.

For patients with moderate to severe ARDS, prone positioning remains a cornerstone of EVIDENCE-BASED care. Prone therapy improves oxygenation, promotes more uniform lung ventilation, reduces ventilator-induced lung injury, and has been shown to improve outcomes when applied appropriately.

Prone therapy and verticalization therapy are not competing interventions, they are complementary tools that support patients at different points along the continuum of critical care recovery.

🔹 Prone Therapy
Used primarily in the acute phase of respiratory failure, prone positioning helps improve oxygenation, recruit alveoli, and reduce ventilator-induced lung injury in patients with severe ARDS. It’s a lifesaving intervention focused on stabilizing pulmonary function during the most critical moments of care.1

🔹 Verticalization Therapy
As patients begin to stabilize, the clinical focus shifts from survival to recovery. Verticalization therapy helps reintroduce gravitational loading and early mobility — supporting pulmonary hygiene, cardiovascular conditioning, neurological stimulation, and musculoskeletal preservation while patients may still be mechanically ventilated.2

🔹 The Pronova-O2® enables both prone, rotational therapy and introduces the initial stages of weight bearing activity through the reverse Trendelenberg feature at 17 degrees which has been shown through tilt table studies to equal approximately 30% weight bearing.
