Most people are familiar with orthopedic physical therapy - this is the treatment of dysfunction of bone,

muscle, joint, tendon, ligament, and nerve tissues which can all generate pain and disability. Pelvic PT is less well known, even by the medical community. As pelvic PTs we are qualified to treat pelvic floor dysfunction, including urinary and fecal incontinence, postpartum issues like diastasis recti and prolapse, and pelvic pain. The best time to prepare for recovery is BEFORE birth.

Have you ever tried to activate a muscle after an injury? Anyone who has had an ACL repair, fracture, or even a sprained ankle knows that trying to learn to control muscles after injury can be challenging. Many muscles will shut off after trauma, while others will spasm to compensate. 

Nerve inhibition or damage can also reduce your ability to engage and strengthen injured muscles as they recover.

With vaginal birth your muscles experience severe stretch and strain, and 80% of first time birthers will tear to some degree. That trauma may come with nerve inhibition that makes restoring your pelvic floor REALLY HARD postpartum, especially if you never knew how to find it before!

C-section does the same to your deep abdominals - cutting through all those layers often inhibits muscle function and can make it very hard to re-activate your core post-birth. (Not to mention your deep abs were SO stretched out at the end they don't even know where they are anymore.)

Postpartum clients always tell us they WISH they had come in during pregnancy to learn the BASICS and have a concrete plan for optimal recovery post-birth.

The time to prepare for postpartum is DURING PREGNANCY, friend!

Preparing your Pelvis for Labor! Two Tips:
1. Labor is "uncomfortable". One way to prepare is to PRACTICE breathing and relaxing INTO discomfort, in different positions.
👉Consider all the positions your doula, birth educator, or "Dr. Google" may have suggested.
Can you easily get in and out of those positions?
Can you HOLD the positions for 2-3 minutes while breathing, over and over, for 8-24 hrs?
Or do you feel strain, tension and fatigue in those positions?
👉Working on this now will help you feel COMFORTABLE using all those positions while also mentally managing your contractions. First time labors can be loooong (mine was 36+ hrs!). Your muscles need endurance to move through and hold varied positions over a long time of work.
Let's build your strength and endurance now so these positions feel EASY and don't add to the stress of labor!

2. Body balancing for fetal descent 
The muscles, joints and bones of the trunk, pelvis and hips surround and impact the uterus. Imbalance in these structures can place pressure and strain on the uterus making it harder for baby to descend smoothly in the optimal position. Practicing specific stretches to address your specific imbalances as well as general pelvic and PELVIC FLOOR mobility techniques can minimize this problem and ease baby more swiftly down the path!

What is it like to "practice push" with a SHEFit PT?
Typically we use manual feedback - either internal vaginal feedback or external with a hand on the perineum, depending on your preference.
1. We ask you to push and see what happens.
2. We provide feedback and direction to increase your ability to push effectively, without contracting your pelvic floor or breathholding.
3. We play with cues, visualizations, and even vocalizations to see what gives you the most power and control!
4. We have you move into different positions - back-lying, sidelying, hands and knees - to see what positions are most effective for YOUR body.
5. We review with you the strategies that worked best and send you home with some things to practice or consider in your birth plan!

Prepare your Pelvis to PUSH!
We've discussed stage 1 of labor. Now for stage 2, where you actually get to PUSH that baby out! 🤰
First, if you are told your pelvis is "too small" to have a vaginal birth, know that this is an "educated guess".
It's okay not to jump into surgery based on a guess, but this is a decision for YOU to make with your birth team.
For those desiring a vaginal delivery, PREPARING for pushing can both build confidence and reduce your risk of perineal tears and other birth injuries!

As Pelvic floor PTs, we CAN:
- test and optimize the mobility of your pelvic joints
- test AND TRAIN your pelvic floor muscle mobility and efficiency for pushing!
- educate you on optimal positions to prevent perineal tearing and protect your pelvic floor
- help you find the most efficient pushing positions for YOUR body

Back-lying, while the traditional position, is NOT preferable for protecting your pelvic floor; it actually carries the highest risk of severe tears.
Positions like side lying, hands and knees, or squatting are preferable, but your options may be limited if you have pain or weakness in those positions.

- we can help ensure you are physically able to labor in preferred positions by eliminating musculoskeletal pain and training your strength for labor!

Regardless of what position you push in, you need to be able to bear down and LENGTHEN your pelvic floor - this is harder than it sounds for many people! Your conscious control of lengthening the pelvic floor is even MORE important if you have an epidural, which may cloud your ability to feel what's happening in your pelvic floor.

Through pregnancy and especially in the 3rd trimester, we can assess/enhance the mobility of your pelvis along with your ability to control, activate and relax your pelvic floor to help you maximize your pushing power and efficiency. Moreover, we train this in any/all positions you might choose during birth to allow baby to pass most easily!

Good stability or control of the pelvis is crucial in preventing prenatal aches and pains including hip pain, back pain, sciatica, and more.

So how do we assess whether this is a factor for YOU?

PTs have lots of tests and measures we use to get a full picture of what's causing your particular problem. The single leg stance test or "march test" is just one measure we use to assess your body and your movement patterns.

This test is helpful if you have pain, and it's helpful for PREVENTION! If you know you have poor control on your right leg in your first trimester, we can give you exercises to improve it and prevent the hip pain from coming on in the second or third trimester!! 🤯
