situs inversus totalis (SIT)
left-sided acute appendicitis
completely mirrored anatomy

CT confirmed the diagnosis, and laparoscopic appendectomy was successfully performed using modified surgical positioning.

Takeaway: Common diseases can present in uncommon ways when anatomy is reversed.
