We’re proud to have brought our Healthcare for Homeless program to the Morgan Hill Senior Resource Fair at the Community and Cultural Center! Our team connected with seniors, provided health screenings, and shared essential resources to support our aging community.

It was inspiring to collaborate with partners like Sourcewise ,Kaiser Permanente, Centennial Recreation Center, and the Senior Advisory Committee—all working together to make a difference.

We were thrilled to attend alongside our colleagues from the Santa Clara Behavioral Health Community Association and the California Behavioral Health Association Together we learned, shared ideas, and explored opportunities for growth and strengthen care coordination across communities.
