Some of the most important work we do isn't always easy to see.

It's the phone call between team members before an appointment. The conversation with a family caregiver who just needs someone to listen. The therapist, nurse, social worker, driver, dietitian, physician, and so many others working together behind the scenes so one person can continue living safely at home.

Because at West PACE, we don't just care for appointments—we care for people.

Every plan is built around what matters most to each participant, with caregivers, families, and a community of support walking alongside them.

That's what whole-person care looks like.
