Menopause doesn't start at 50. It can begin in your late 30s or early 40s — sometimes earlier.
Hot flashes, irregular periods, brain fog, mood shifts, sleep disruption, weight changes around the midsection... these aren't just "stress" or "getting older."
They're hormonal. They're real. And they're treatable.

Not All Doctors Are Trained for This
"Obesity medicine is a specialty."
Obesity involves complex hormonal, metabolic, and psychological systems.
An obesity specialist has dedicated training in exactly this — from thorough evaluation to advanced diagnostics to GLP-1 medications to behavioral strategies.
One-size-fits-all advice doesn't work because obesity isn't one-size-fits-all.

MORE THAN 90% OF PEOPLE DO NOT GET CLOSE TO ENOUGH FIBER.
One medium apple has about 4 grams of fiber. That is only around 15–20% of daily needs.
Fiber impacts far more than digestion: • Cholesterol • Blood sugar • Gut microbiome • Fullness • Metabolic health
BUT not all fibers work the same way.
Sometimes better health starts with understanding the basics differently.

I have heard people say “I hate cholesterol pills.”
I also know many people should have NOT taken on these medications.
And many time, people take statin without even know
- why
- what are the target? is it effective?
- what to follow up...

And honestly?

Part of that reaction exists because medicine is too often practiced using a “good public health default” instead of a true personal decision.

Everyone is different.

Genetics.
Epigenetics.
Family history.
Inflammation.
Diabetes.
Blood pressure.
Metabolic health.
Lifestyle.
Stress.
Sleep.
Diet.
Response to statins.
Tolerance to medications.
Progression of plaque over time.
Even the same LDL number can mean very different things in different people.

One patient may truly benefit from early aggressive treatment.

Another may need lifestyle changes, more data, more discussion, more follow-up before making that decision.

This is why cholesterol care should never become:
“Your LDL is high. Here’s a pill.”

But it also should not become:
“Statins are bad for everyone.”

Both extremes miss the point.

The real goal is understanding:
Who is actually at risk?
Why?
How aggressive should treatment be?
And what is the right target for THIS patient?

That conversation takes time.

And unfortunately, time is becoming the rarest thing in healthcare.

YOUR HEART DOCTOR AND GI DOCTOR BOTH RECOMMEND FIBER.

But they may NOT be talking about the same thing.

Some fibers help:
• Cholesterol
• Blood sugar
• Heart health

Others mainly help:
• Constipation
• Stool bulk
• Digestive movement

This is why “high fiber” is not one-size-fits-all.

The type of fiber matters.
Your gut matters.
Your health goals matter.

Healthcare becomes different when the conversation becomes personalized.

A patient should have started cholesterol treatment a long time ago.

But he delayed.

Not because he didn’t care.

Because nobody ever really explained things to him.

I don’t blame him.
And honestly, I don’t blame his doctors either.

They never had enough time for a meaningful conversation.

Never enough time to fully understand his real cardiovascular risk.
Especially his family history.

His dad died from a heart attack.

That alone changes the conversation.

But many patients never get the time to connect those dots.

He also never had the chance to do advanced testing like Lp(a) or ApoB.

Again—not because nobody cared.

But because navigating insurance approvals, referrals, paperwork, and “do you really need this test?” becomes exhausting.

And many people never realize:
self-pay labs are often simple, transparent, and surprisingly affordable.

Then when he finally decided to take prevention seriously, he was referred to a GI specialist only because he had a “history of liver condition.”

Any MD/DO could spend one minute realizing it was not a real barrier to treatment.

Instead, he was told to wait months for referral clearance.

One short virtual conversation later:
all his questions were answered.

We started treatment the same day.

This is the real problem in modern healthcare.

Not lack of science.
Not lack of medication.

Lack of time.
Lack of access.
Lack of meaningful conversations before it’s too late.

What if anxiety, low mood, and brain fog are not only happening in the brain…

But also in the gut?

The gut and brain constantly communicate with each other.

In fact, a large portion of neurotransmitters involved in mood regulation are connected to the gut environment and microbiome.

When gut health becomes disrupted, people may experience:
• Anxiety
• Low mood
• Brain fog
• Poor sleep
• Fatigue
• Food cravings
• Chronic inflammation

This does NOT mean every mood symptom is “just the gut.”

But the body is deeply connected.

And sometimes mental wellness requires a broader conversation about:
Sleep.
Stress.
Nutrition.
Inflammation.
Hormones.
Gut health.
Lifestyle.
And long-term support.

a patient texted me at 6 PM. we were able to meet virtually next day.
spent nearly an hour reviewing many things from symptoms and lifestyle to wearable biometric data and functional testing.
it is not uncommon in my practice. It is our standard.
That is Direct Care. New level of care. Medicine without border. without barriers. no insurance or anyone dictate how much time a patient deserves, what to discuss, what data, what test, what treatment they deserve. they deserve better.

FIBER LOWERS CHOLESTEROL. BUT probably not the way you think.
The fiber helping your heart is often NOT the same fiber helping constipation. Some fibers form a gel-like substance (call: soluble fiber) that helps reduce LDL cholesterol.
Others mainly add stool bulk and improve bowel movement.

Your cardiologist and GI doctor may both say: “Eat more fiber.” BUT they may mean very different fibers.
Nutrition is more nuanced than most people realize. Want a more personalized approach to gut and metabolic health?

If you’re a woman in your 40s or 50s and your sleep, mood, weight, cravings, and hormones all changed at the same time…

Your gut may be part of the conversation.
Hormones and gut health have a two-way relationship.

Menopause and perimenopause can disrupt the gut microbiome.

And gut dysfunction can worsen hormonal imbalance.

This is why many women in midlife suddenly experience:
• Bloating
• Food cravings
• Brain fog
• Fatigue
• Poor sleep
• Mood changes
• Weight gain
• Increased inflammation

Because estrogen is not only produced.

It also needs to be metabolized properly.

And part of that process happens through the gut.

When the gut microbiome becomes disrupted, estrogen metabolism may also become disrupted.

The result?

The body can feel like everything is changing at once.

This is why menopause care should never become:
“Here’s a prescription. See you in 6 months.”

Women deserve a broader conversation about:
Hormones.
Gut health.
Inflammation.
Sleep.
Stress.
Nutrition.
Muscle health.
Metabolism.

Because the body does not work in isolated systems.
Gut Health Testing is part of our holistic evaluation.
