I’m so grateful for this patient letting us share her images because this is the perfect lesson in anatomy. Her concern was the lower face - which is not uncommon for us to hear. Without knowing, you would assume that’s where we’re going to place the filler. And sometimes, that wouldn’t be wrong. But every time requires a thorough assessment of what is actually happening with the anatomy of aging. It’s never anything personal - it’s just how we were designed.
Heaviness or folding in the lower face is often an *symptom* of lost volume either above it, outside of it, or both. So we’ll often look to the apples of the cheeks, across the cheek bones, or in front of the ear for volume loss or hollowing. For this patient, we thought we could get her the most bang for her buck by focusing on the apples of the (anterior) cheek and the improvement was pretty immediatel. This is such a perfect lesson that slowing down and having a conversation about why we’re seeing what we’re seeing is immeasurablely important. With the knowledge of what changes are affecting the fat pads, bones, skin, and ligaments, we were able to curate a plan that worked for her concerns while still respecting the anatomy. And at the end of the day, that always leaves us with the best feeling! 🤓
