Your Veins & Lymphatic System Are More Connected Than You Think

Swelling in the legs isn’t always caused by just the veins or just the lymphatic system. These two systems work closely together to move fluid out of your legs and back into circulation.

When the veins aren’t working properly, increased pressure can cause excess fluid to leak into the surrounding tissues. Your lymphatic system steps in to help remove that extra fluid.

But what happens when it has to keep doing that day after day?

Eventually, the lymphatic system may become overwhelmed or damaged, leading to persistent swelling. When chronic venous disease and lymphatic dysfunction occur together, this is often referred to as phlebolymphedema.

πŸ”Ž Signs may include:

β€’ Persistent leg, ankle, or foot swelling
β€’ Swelling that becomes harder or more difficult to reduce
β€’ Heaviness or tightness
β€’ Blisters, weeping, or fluid leaking from the skin
β€’ Skin thickening or hardening
β€’ Skin discoloration
β€’ Recurrent cellulitis or skin infections
β€’ Slow-healing or recurrent wounds

🩻 Why can a vascular ultrasound be important?

If someone has chronic or unexplained leg swelling, it may be important to evaluate the venous system, not just treat the swelling.

A comprehensive vascular ultrasound can help look for:

🩸 Venous reflux- Are the valves allowing blood to flow backward?

🩸 Previous DVT or post-thrombotic changes- Has prior clotting damaged the veins and affected drainage?

🩸 Venous obstruction- Is something interfering with normal blood return?

🩸 A problem higher up- In certain patients, evaluation of the iliac veins and IVC in the abdomen/pelvis may provide another piece of the puzzle.

πŸ’‘ The takeaway:
Lymphedema and venous disease aren’t always separate problems. Sometimes treating swelling effectively means understanding why the fluid is accumulating in the first place.

Don’t just look at the swelling. Look for what may be contributing to it.

Sometimes the Problem in Your Legs… Isn’t Actually in Your Legs.

When someone has leg swelling, heaviness, pain, varicose veins, skin changes, or venous ulcers, we often start by evaluating the veins in the legs.

But what happens when the leg ultrasound doesn’t fully explain the symptoms?

πŸ” Sometimes we need to look higher.

The veins in your legs eventually drain into the larger veins in your pelvis and abdomen. If blood flow is narrowed, compressed, obstructed, or otherwise abnormal higher up, it can affect how efficiently blood drains from the legs.

🩸 An abdominal/pelvic venous ultrasound may evaluate:

IVC (Inferior Vena Cava)
The major vein responsible for returning blood from the lower body back toward the heart.

Iliac Veins
These large pelvic veins receive blood from the legs. Abnormal flow, narrowing, obstruction, or compression can contribute to lower-extremity venous symptoms.

Venous Flow Patterns
Doppler waveforms provide important clues. Changes in normal respiratory phasicity or significant differences between sides can sometimes suggest a problem farther upstream.

⚠️ When might we want to look higher?

Persistent or unexplained leg swelling, especially when one leg is significantly different from the other; recurrent or difficult-to-heal venous ulcers; recurrent DVT; extensive collateral veins; unusual venous Doppler flow patterns; or symptoms that simply don’t match what we’re seeing in the legs.

πŸ’‘ The important part?

A good vascular ultrasound exam isn’t just about finding an abnormality. It’s about asking:

Does this finding actually explain the patient’s symptoms and if it doesn’t, where else should we look?

Sometimes getting to the root of a lower-extremity venous problem means looking beyond the legs.

You don’t have to have visible varicose veins to have venous insufficiency.
