ings attached. If you’re already enrolled in TRICARE®Prime you can switch to USFHP by simply filling out the DD Form 2826 application. Your coverage switches over the day we receive your application. If you are an Active Duty Family Member and currently covered under TRICARE®Prime, you can switch to USFHP by calling 1-800-585-5883. If new to TRICARE®Prime, your coverage starts the first day of the month if we receive your application and any applicable enrollment fees by the 20th day of the prior month.
