Easily the number one reason I see for ongoing shoulder pain, especially shoulder pain that persists despite treatment, is a failure to account for the neck as a source of symptoms.
Well over half of the shoulder pain patients I see have symptoms that are driven by the neck, and require treatment directed at the neck for pain to resolve.
By far the best way to determine this is a well done McKenzie Method assessment.
