PAD doesn't always introduce itself with leg pain.

Sometimes the problem showing up in your foot didn't start in your foot.

PAD reduces blood flow through the arteries that carry oxygen-rich blood toward the legs and feet. That's why changes that appear to be foot problems can sometimes point to a circulation problem farther upstream.

Sometimes saving a foot starts much farther upstream.

For Dr. Stratton Danes, choosing vascular surgery came down to something he still appreciates 21 years later: no two days, or patients, are quite the same.

During training, a vascular professor helped him see the extraordinary breadth of the specialty. Vascular surgeons treat arteries and veins throughout the body, with care ranging from limb preservation, stroke prevention and aneurysms to helping someone feel better after treatment for varicose veins.

That range of care is matched by something equally important to Dr. Danes: relationships.

When a nervous patient walks through his door, his first priority isn't surgery. It's understanding.

“A great deal of my patients think they need surgery, but most patients just need information and a care plan that can get them on the correct path to better health.”

And when surgery is necessary, he wants patients to understand what to expect and know he'll be there through their recovery.

Outside the office, life has a distinctly South Shore vibe. An ideal day starts early with his dogs in the cranberry bogs, followed by tending his vegetable garden, a beach walk with his wife, golf or tennis, then cooking something special and enjoying a quiet evening on the porch.

And if he weren't a vascular surgeon? Having grown up in a restaurant family, Dr. Danes could picture running a farm supplying his own farm-to-table restaurant. Or becoming a surfing instructor in Costa Rica.

But after more than two decades in vascular surgery, what matters most is right here.

“I am a member of the community in which I work, and I take that responsibility very seriously.”

For Dr. Danes, getting to know his patients and their families, and helping his community through advanced vascular care, is what continues to make the work meaningful.

Ever notice your leg discomfort seems to have a schedule?

You start walking. You're fine.
Then, after a certain distance, the aching, cramping, heaviness or fatigue begins.
You stop for a few minutes.
It gets better.
You start walking again.

And there it is again.

That pattern can be an important clue to peripheral artery disease (PAD).

When narrowed arteries aren't able to deliver enough blood to working muscles, symptoms may appear during activity and improve with rest—a symptom known as claudication.

And here's the not-so-obvious part:

It can happen at a surprisingly predictable level of activity.

Same hill.
Same distance.
Same discomfort.

It's easy to blame being out of shape, getting older, arthritis or simply “doing too much.”

But when your legs repeatedly tell you, “this far and no farther,” it may be worth listening.

This PAD Awareness Month, we're sharing some of the lesser-known clues and facts about a disease that affects far more than the way you walk.

Your walking distance isn't just a number. Sometimes, it's information.

For Dr. Andrew Lazar, vascular surgery stood out from his very first surgical rotation in medical school.

He was fascinated by vascular physiology, but it was the breadth of the specialty that really drew him in. He describes vascular surgeons as something like the “firefighters of the hospital;” called upon in all kinds of situations, from planned procedures to emergencies.

He also appreciates a specialty that embraces new technology while never losing sight of the foundational techniques that vascular surgery is built upon.

But Dr. Lazar’s approach to patients has been shaped by something more personal: he’s experienced healthcare from both sides of the exam room.

Those experiences taught him how important it is for patients to understand what’s happening and know that their physician is there to help.

Sometimes that means breaking down a complex problem into something easier to understand. Sometimes it means performing a procedure or surgery. And sometimes, he says, the best way he can help is recognizing that the problem isn’t vascular and connecting that patient with someone who can.

What makes it worthwhile?

“Each time we are able to help a patient get back to the life they want reminds me of how much of a difference we can make.”

Outside medicine, that idea of enjoying life hits close to home. Family comes first. Weekends are spent with his wife, children and their dog, getting outside, exploring something new, or simply being together.

You take care of everyone else. Who’s taking care of you?

The kids. Your spouse. Your parents. Your grandkids. Work. The house. The appointments. The never-ending list.

Somehow, you keep getting moved to another day.

That can include the aching, heaviness, swelling, cramping, or tired, restless feeling in your legs that you’ve learned to live with.

But those symptoms may be signs of venous insufficiency, and varicose veins can be more than a cosmetic concern.

At The Vascular Care Group, getting answers doesn’t have to turn your life upside down. We offer convenient, office-based vein care designed to fit into real life, with many treatments taking less than an hour and requiring little to no downtime.

Because taking care of yourself doesn’t mean you stop taking care of everyone else.

It just means you finally put yourself on the list, too.

Your legs have been waiting. Schedule a vein evaluation today.

There are things you learn in nursing school.
And then there are things you learn from caring for thousands of patients.

You learn to notice the foot that's a slightly different color.
The pulse that's harder to find.
The swelling that wasn't there yesterday.
The wound that's changing.
The patient who says, “I'm fine,” but seems a little more worried than they did five minutes ago.

𝙑𝙖𝙨𝙘𝙪𝙡𝙖𝙧 𝙣𝙪𝙧𝙨𝙚𝙨 𝙣𝙤𝙩𝙞𝙘𝙚.

They assess. They educate. They anticipate. They advocate. They reassure. And they bring highly specialized vascular knowledge to the bedside, procedure room and clinic every single day.

But perhaps most importantly, they never let the disease become more important than the person who has it.

This Vascular Nurses Week, we're celebrating the exceptional nurses across The Vascular Care Group who make comprehensive vascular care possible.

We talk about health a lot around here.

This weekend, the UN-official end of summer, we're keeping it simple.

Spend some time with people you love. Go somewhere that makes you happy. Put the phone down for a while. Stay for the sunset.

Summer isn't quite finished yet.

Neither are we.

PAD Awareness Month is about recognizing the disease. But awareness alone doesn’t move medicine forward. Research does.

At The Vascular Care Group, treating peripheral artery disease is only part of our commitment to patients with PAD. We also believe we have a responsibility to help advance how vascular disease is understood and treated.

That commitment is reflected in our participation in the XTRACT Registry, a nationwide post-market study evaluating the real-world performance of the Rotarex™ Catheter System in the treatment of PAD.

Recently, Dr. Scott James successfully enrolled TVCG’s first two patients in the XTRACT Registry, an important milestone for our team and another contribution to the growing body of evidence shaping vascular care.

The XTRACT Registry is designed to enroll up to 600 patients at as many as 100 sites across the country, collecting real-world clinical data to evaluate the safety and effectiveness of the Rotarex system and help inform PAD treatment strategies.

But XTRACT is also part of a much bigger story at TVCG.

Through our affiliation with Vascular Breakthroughs, The Vascular Care Group has helped create an environment where clinical research and everyday patient care intersect. Since 2012, Vascular Breakthroughs has managed dozens of clinical trials and helped bring vascular research, and access to emerging technologies, beyond traditional academic medical centers and into the communities where patients receive their care.

That means our physicians aren't simply keeping pace with advances in vascular medicine.
They're helping generate the evidence that moves the field forward.

This PAD Awareness Month, we're proud to recognize Dr. James and the entire research team for another step in that mission, and the patients whose participation makes progress possible.

Here's a strange fact about peripheral artery disease:

You can have it without the symptom that is most associated with it.

An estimated 20–59% of people with objectively confirmed PAD may be completely asymptomatic. And some people who report no exertional symptoms develop them during objective walking testing.

Which means:

“My legs don't hurt” isn't the same thing as “my circulation is fine.”

Thirty years ago, vascular surgery looked very different than it is today, and Dr. Daniel Gorin had a front-row seat for its transformation.

When he trained, vascular surgery was still part of general surgery. “Whenever any other surgeon got into trouble in the OR, they called a vascular surgeon for help.” The complex decision-making and technical skill drew him to the specialty.

Over his career, Dr. Gorin has helped advance vascular care as many of the large open operations have given way to minimally invasive, catheter-based treatments often performed outside the hospital. Being at the forefront of that evolution ultimately led him to join with others to found The Vascular Care Group in 2020.

But his perspective comes from being a patient, too.

“Being a patient stinks.”

As a surgical resident, Dr. Gorin contracted a rare form of tuberculosis, spending seven weeks in the hospital and nearly six months away from work.

“No one puts having a vascular procedure at the top of their wish list. Our goal is to make it as easy and comfortable as possible, so you can get on with the important job of living your life.”

Outside the office, this Cape Codder loves all things water, boating, fishing, and remote travel. But there’s another side to him that patients might not expect: In 1994, during his vascular fellowship, he co-founded The Works: Boston's Favorite A Ca****la Singing Group, an eight-member a ca****la group that still rehearses and performs today.

If he weren’t a surgeon? Maybe a tropical fly-fishing guide playing in a beach-bar band at night, or, more likely, working at a startup trying to solve the next big problem.

Apparently, sitting still was never really an option.
