Nobody wants to make the locum call on day 30. It feels like admitting the permanent search is not going well.

Here is the trouble with waiting. By day 90 your remaining providers have carried the extra volume for three months, the revenue is gone and is not coming back, and the permanent search is often sitting where it was in week two. The call gets made anyway. It just gets made from a worse position.

Bringing in coverage early is not giving up on the hire. It keeps the panel intact and takes the weight off the people carrying it while you find the right person.

A recent AAPPR benchmarking report has two takeaways worth the read, regardless of where your facility sits in the rankings.

First, time-to-fill is no longer the right primary metric for the hardest specialties. Cost per vacancy day has overtaken it because the cost moves with revenue, while time-to-fill plateaus once you cross 180 days.

Second, the gap between the top quartile and median time-to-fill has widened, not narrowed. That means the operational fixes that worked in 2022, including more recruiters, more job board spend, and more LinkedIn outreach, are returning less now than they did then.

From our seat, the systems that have closed the gap have done it by building parallel, specialty-specific pipelines, not by adding more general recruiting capacity.

Three places candidates leave money behind on a first physician contract. (Watch below.)

1. The call schedule. The base looks identical until you map 1-in-4 against 1-in-6 across three years.
2. CME and license fees. Four figures a year, almost always negotiable.
3. The non-compete radius. A 25-mile radius reads fine until you have to move when the relationship doesn't last.

These three line items have probably moved more dollars in physician contracts over the last 30 years than base salary itself.

Save this for when you see your first contract.

Workforce shortages are not moving uniformly across the country.

Per recent KFF analysis: rural and frontier counties are seeing the steepest physician deficits, while metro areas show modest improvement. The gap matters for hospital systems, FQHCs, and community health partners trying to staff their care teams.

What this means for our recruiting work: same national demand, very different supply by region. We are tracking it closely on behalf of our partners.
