Exciting News!

Rock Primary Care and Wellness has teamed up with Nsight Health to provide regular monthly check-in on our patients that have chronic conditions such as hypertension (Hight blood pressure) and Diabetes. They will provide some patients with home devices to monitor their blood pressure, blood sugar, oxygen levels and weight.

There is no complicated equipment- just easy to use devices that fit seamlessly into your daily routine.
The remote patient monitoring program will help me as your provider to create a more effective care plan and quickly address any concerns.
