In the United States, the crisis doesn’t always end when the ambulance arrives.

A single ambulance ride can cost $2,500 or more, and that’s often just the beginning. Emergency room charges, diagnostic tests, physician fees, and follow-up care can quickly add up. What might be a short trip to the hospital can lead to months of medical bills.

Because of these costs, some people hesitate before calling for help—wondering how far the hospital is, considering alternatives, or hoping the situation isn’t serious enough to require emergency care.

For many, the financial uncertainty surrounding healthcare has become part of the emergency itself, highlighting ongoing debates about affordability and access within the American medical system.

Why does a tonsillectomy hurt so much?

When a patient has their tonsils removed because of recurrent strep throat or chronic tonsil infections, the tonsils are taken away from the muscles they sit against in the throat.

No matter what surgical method is used, the muscle underneath is exposed and irritated during the procedure.

That same muscle helps with swallowing, talking, breathing, and normal throat movement. This is why pain after a tonsillectomy is common, especially when swallowing.

Tonsillectomy recovery can be uncomfortable, but understanding why the pain happens can help patients and parents know what to expect during the healing process.

A person can live for years with:
• “Sometimes it goes up”
• “Higher in the morning, then normal”
• “Tests seem fine”

But during this time, the blood vessels are already losing elasticity.
It’s important to understand:
Not every increase in blood pressure = “incurable hypertension”
But not every “it sometimes rises” is safe either.

Very often, this is caused by:
• Morning blood pressure spikes
• High diastolic (lower) pressure
• Resting pulse of 85–95
• Stress and poor sleep
• Excess weight or insulin resistance
In such cases, simply increasing the dose of medication is not always the solution.

Case of a foreign re**al body, a large cucumber, penetrating the bowel and needing an urgent laparotomy!!
The first step in the evaluation and management of a patient with a re**al foreign body is to determine whether or not a perforation occurred. When a perforation is suspected, it should be determined as soon as possible whether the patient is stable or unstable. Hypotension, tachycardia, severe abdominopelvic pain, and fevers are indicative of a perforation. If there is free air or obvious peritonitis indicating a perforation, then the patient needs immediate resuscitation with intravenous fluids and broad-spectrum antibiotics followed by an operation. A Foley catheter and nasogastric tube should be placed, and appropriate blood samples should be sent to the laboratory. If the patient appears stable and has normal vital signs and a perforation is suspected, a computed tomographic (CT) scan often helps determine if there has been a re**al perforation.

The majority of cases of re**al foreign bodies are the result of insertion during erotic activity. Typically found objects are vi*****rs, di**os, light bulbs, candles, shot glasses, and bottles. Some re**al foreign bodies may have been initially swallowed but later present with complications in the re**um. Patients may be very embarrassed to disclose the circumstances regarding the foreign body insertion, and there may have been multiple attempts at self-removal.

A sticker from a U.S. public restroom became a global design fail and a textbook example of bad graphic design.

The idea was simple: remind people to wash their hands more often. But instead of clearly showing handwashing, people saw pretty much everything except that.

Ironically, it ended up working. The design was so aggressive and uncomfortable that visitors interpreted it as a warning and actually washed their hands more often.

Fish Hook being removed from the eye! 👁
This young fisherman presented to the emergency department with severe ocular pain, tearing, and decreased visual acuity after a barbed fish hook recoiled and struck his right eye.
The hook was embedded in the inferotemporal globe with associated subconjunctival hemorrhage and a full-thickness scleral laceration.

He was taken urgently to the operating room. The removal was performed in a retrograde fashion to minimize additional tissue disruption. Rather than advancing the barb forward, which would enlarge intraocular damage, the hook was gently backed out along its original trajectory through the entrance wound. The primary scleral laceration was slightly extended in a controlled manner to allow safe extraction of the barb without further shredding of scleral fibers. Meticulous wound repair followed, including layered closure of the sclera and conjunctiva, anterior chamber reformation, and intracameral antibiotics.

Barbed fish-hook injuries are particularly hazardous because the barb prevents simple withdrawal and risks exacerbating retinal or uveal trauma. Prompt surgical management, atraumatic retrograde removal, and watertight globe repair are critical to preserving vision.🏥

A cancer treatment that helps the immune system fight back without traditional chemotherapy?

Anktiva is a newly FDA-approved immunotherapy for certain patients with non–muscle invasive bladder cancer whose disease has stopped responding to the standard bladder treatment called BCG. Instead of directly killing cancer cells like chemo, Anktiva is designed to boost the body’s own immune system.

In simple terms, Anktiva:

➕Activates “natural killer” cells and killer T cells, the immune cells that recognize and attack cancer

➕Helps create “memory” T cells so the immune system can keep watching for cancer cells over time

➕Is given directly into the bladder (not as a pill) and used with BCG, not in place of it 

Anktiva is not for every type of cancer and not for every patient. Like all cancer therapies, it can have side effects, including urinary symptoms, fever, and infection, and it is only available by prescription through oncology and urology specialists.
