Nascentia Health is seeking a full-time ISNP Clinical Coordinator (RN) to join our team and work on-site at a designated skilled nursing facility in Roscoe, NY!

In this role, you’ll serve as an important connection between members, families, facility staff, medical providers, and the Nascentia Health team by helping coordinate care, support smooth transitions, address changes in condition, and improve health outcomes.

Three or more years of home health or long-term care experience is strongly preferred.
