You’re not stuck.

In many cases, what feels like a lack of progress is actually a mismatch between the approach being used and the underlying physiology driving weight and metabolism.

I regularly see patients who are doing everything they’ve been told, making changes, staying consistent, and still not seeing the results they expect. That’s not a failure of effort. It’s an indication that the approach needs to be more aligned with their individual biology.

When we shift the strategy and take a more personalized, medically guided approach, outcomes begin to change in a meaningful and sustainable way.

You don’t need to have everything figured out to begin.

In my practice, most patients don’t come in with a clear roadmap. They come in after trying multiple approaches, knowing something still isn’t working, but unsure what the next step should be.

That uncertainty is normal.

What makes the difference is having structured, physician-guided care early in the process. In bariatric and metabolic medicine, that initial evaluation can significantly impact long-term outcomes.

In clinical practice, patients who focus on repeatable, sustainable behaviors are significantly more likely to achieve durable results than those attempting to maintain a “perfect” plan.

This principle applies across all levels of care, including lifestyle intervention, medical management, and surgical treatment.

The body responds to what is done consistently over time.

Many people are surprised to learn they may qualify for bariatric surgery.

One of the most common misconceptions I encounter is that there is a "perfect candidate" for surgery. In reality, eligibility is determined through a comprehensive evaluation that looks at the whole patient, not a single number on a scale.

When I evaluate a patient, I focus on three primary areas. First, we determine whether surgery can be performed safely based on their overall health. Second, we review their medical history to better understand their experience with weight management and identify the most appropriate path forward. Third, we discuss support systems, because long-term success is rarely achieved in isolation.

The evaluation process is not a test. It is an opportunity to understand your health, your goals, and the treatment options available to you.

There is no perfect candidate and no perfect starting point. There is simply the decision to begin the conversation.

I just got back from the ASMBS national meeting and want to share something relevant for anyone researching their weight loss options right now.

The conversation among bariatric surgeons has moved on from technique debates. What we are all focused on now is how GLP-1 medications work alongside surgery, not instead of it, and what the long-term data is telling us about sustained outcomes.

The field is landing on personalized metabolic care. No more one-size-fits-all. The right plan is the one built around you specifically, not the trending medication or procedure.

I spent the last week at ASMBS, one of the most important conferences in bariatric and metabolic surgery, and I came home with a lot to think about.

Here are the three things that are going to stay with me.

First, the GLP-1 conversation has matured. Surgeons are no longer debating whether these medications are effective. That question has been answered. What we are focused on now is how to thoughtfully combine GLP-1 therapy with surgery to build care plans that are individualized, sequenced correctly, and built around each patient's specific needs and goals.

Second, mental health and long-term support have moved to the center of the conversation, not the margins. The best outcomes happen when patients have comprehensive support teams around them, and when surgeons take ownership of long-term care rather than considering their job done once the procedure is complete.

Third, and this one is for my patients specifically: the research is clear that the people who do best over time are the ones who stay connected to their care teams. Who stay curious. Who continue to show up, ask questions, and treat this as an ongoing process rather than a finish line they crossed.

Most patients I see come in after years of trying different approaches on their own. By the time they reach this point, they’re not starting over, they’re starting with more clarity and finally the right support.
