I will be asking everyone new and old to fill out a new intake form.

Intake forms are very important to help the therapist understand the contraindications that might hinder your progress with your massage.

I’m happy to say that I have plenty ready for you to come by the office anytime this week to get it before your visit, I can email you a copy, or please plan to come a little early or plan to stay a little late the day of your next massage.

Also, this will help providing me with information of addresses and birth dates. These will become very important as the year progresses for you as a client and me.

Reminder- just because it looks like a lot of time slots available in one day does not mean I can fill them all. I only have a certain amount of people I can do in a day but I like to offer a selection of times.
