Did you know that nearly 95% of older adults live with at least one chronic illness and almost 80% have two?
Managing multiple medications can be challenging under the best of circumstances. If you factor in increasing age, multiple diagnosis, poly pharmacy, loss of fine motor skills, impaired vision and cognitive decline, this can be a recipe for disaster.

There are options for easier medication management and compliance. We can help.

I read this quote, written by Natalie Justice, a cancer warrior and author.

“Even in the hardest days, it's okay to cry. It's okay to laugh. It's okay to scream, and God can handle all of it. And more than anything, I hope that they feel wrapped in God's love, and remember that hope is still alive even in that moment, on that journey.

As true as those words are for Natalie, they can speak volumes for those that are faced with many illnesses, as well as those that care for them.

There are hard days. Sometimes, more than we can seem to bare. And it is okay to cry. It is okay to scream. It is okay to feel scared or paralyzed with fear. It is okay to be angry and broken. And it is okay to smile and even laugh, when you may otherwise feel like you are carrying the weight of the world on your shoulders. There is always hope. There is support. You are not alone. You do not have to navigate this journey and without help. We are just a phone call away.

What Is a Care Plan?
from: Foothills Care Management Services led by Katrina Kepf
🤔When your or a loved one’s needs start to grow, it can quickly feel overwhelming—doctor appointments, medications, safety concerns, and big decisions all at once. That’s where a care plan comes in. 💙
A care plan is a personalized roadmap designed to support someone’s health, safety, and overall well-being. It takes into account their medical needs, daily routines, home environment, and even emotional and social support.
At Foothills Care Management Services (www.foothillscms.com), care plans are built through comprehensive assessments—looking at the full picture, not just one piece. From there, families receive clear, written recommendations and ongoing guidance to help coordinate care, communicate with providers, and avoid things slipping through the cracks.✨
In simple terms:
A care plan brings clarity, structure, and peace of mind to complex situations—so families can focus less on managing everything and more on being present with the people they love.If you’re feeling overwhelmed trying to juggle it all, you don’t have to do it alone.

A well-designed care plan can make all the difference. 💛
