Might ruffle some feathers but I've seen a lot in 26 years in healthcare aka my opinion that no one asked for...
1. Microdiscetomy maybe fusion as a very last resort because sensation and function were being lost. Seen too many people have issues down the line.
2. I think it's a damn shame that surgeons are not recommended hands on post-op care, total disservice to healing. And therapists not doing hands on work in early recovery along with function and exercise are missing something patients want and need.
3. After surgery, having a relationship with a therapist can make or break your ability to fully get back to your goal activities. It may not be 3x/week but having a place when your plan doesn't work is important.
4. Years?!? consistently going to any provider for the same issue seems like it's not working IMO. I have people come on and off for years but we're usually jumping around or they are in a flare and are coming for a tune-up. I hate needing to always find new people but I'm not in this for you to see me regularly forever.
