Being diagnosed with spondyloarthritis myself, I tend to approach cases like these with a bit of an edge. Here’s how I thought through this one, and ultimately ended up at the same diagnosis I received years ago.

Scleroderma doesn't always start with obvious skin changes. Some warning signs may show up in places you wouldn't expect.

Changes in your fingers, persistent heartburn, trouble swallowing, or unexplained shortness of breath may seem unrelated at first. But these symptoms can sometimes point to a condition that affects more than one part of the body.

Understanding how this autoimmune disease presents is important because early signs can be easy to overlook.

Have you noticed symptoms that seem unrelated but might be connected? I’d be interested to hear your thoughts or experiences in the comments.

If I lined up 100 patients with moderate-to-severe autoimmune or inflammatory disease, a large chunk of them would be offered some type of immune-modifying medication.

I prescribe immunosuppressants, biologics, DMARDs, and other rheumatology medications every week. Why? Because inflammation can cause real damage. Medications like methotrexate, hydroxychloroquine, biologics, and JAK inhibitors can reduce inflammation, prevent joint destruction, protect organs, improve function, and help patients get their lives back.

But as an integrative rheumatologist, I don’t think the conversation should end there. The medication helps control the immune response...but I’m also interested in what may be driving it.

And my goal is to use every tool available to help patients feel and function better.

When I see elevated urine protein levels in a patient, I pause there before moving on. Here’s why.

The biggest barrier to exercise for most people with autoimmune disease has everything to do with trying to figure out what your body can tolerate, especially during a flare.

In a survey of people living with rheumatic disease, fatigue (61%), pain (54%), and swollen joints (51%) were the biggest reasons people struggled to stay active.

Living with autoimmune disease has changed the way I train. I prioritize finding an approach your joints and your immune system can recover from consistently, rather than Instagram’s most effective workout of the week.

That’s how you stay active for years.

Comment RESET if you’d like my 21-Day Autoimmune Reset. It’s the same framework I use with patients to calm inflammation before building back up again.

“Can we reduce my medication?”

It’s a question doctors hear all the time. I’d love for the answer to primarily be yes–but this is why context is important.

How do you know which tests are actually worth doing when you’re dealing with a complex chronic illness?

More testing can provide additional information, but it can also bring higher costs, more data to interpret, and questions about what really matters for patient care.

Personalized medicine isn’t about ordering every test available. It’s about knowing when the findings justify looking deeper and when a more focused approach makes sense.

Dr. Nate Handley and I explore this on the Autoimmune Alchemy Podcast, including how clinicians approach testing when evaluating complex health conditions.

Your genetic test shows a variant. Does that automatically mean you need another supplement?

Not necessarily.

When it comes to personalized medicine, there’s a difference between what your genes suggest could happen and what’s actually happening in your body.

That distinction matters. Without looking at the bigger picture, it’s easy to end up addressing genetic findings that may not need intervention.

On the Autoimmune Alchemy Podcast, Dr. Nate Handley and I explore how genetics fits into a more complete understanding of health—and why a genetic result shouldn’t be the only factor guiding your next steps.

I’ve had a lot of conversations with Sjögren’s patients where we’re managing individual pieces of the disease because our treatment options are still limited.

That’s why I’m following this one closely.

I’d love to have more options to offer these patients. And I’d especially love for the next generation of rheumatologists to inherit a much longer list than the one I started with.
