A little birthday cake, and a good excuse for a quick team photo. 🎂💙

We don’t always manage to get everyone in the same place at the same time, so when we had part of our Vero Beach team together for cake, we took advantage of the moment!

These are some of the faces helping care for our patients, answer the phones, coordinate procedures, keep the office moving, and make ISPI what it is every day.

We’re grateful for this crew — and for the patients and community who continue to trust us with their care.

Why was ISPI created?

When Dr. Esposito founded Interventional Spine & Pain Institute, he wanted the freedom to build a different kind of pain practice.

"I wanted to create a practice where decisions could be made based on what is right for the patient."— Michael Esposito, MD, Founder & Medical Director

That philosophy continues to shape how we care for our patients today.

We believe pain should be treated like any other medical condition: without judgment, with careful evaluation, and with an individualized treatment plan.

Rather than trying to address everything at once, our team works to identify the primary pain generator and take a thoughtful, stepwise approach—providing the care that is appropriate for that patient at that point in their journey.

Being named a 2026 Best of Florida® winner is meaningful to us because it recognizes a practice that was built around that philosophy.

Comprehensive. Compassionate. Advanced.

🔥 Therapy Thursday: What Is Radiofrequency Ablation (RFA)?

One of the most common questions we hear is:
"What exactly is a Radiofrequency Ablation?"
Radiofrequency Ablation (RFA) is a type of injection used to help reduce chronic pain by interrupting pain signals from small sensory nerves.

Think of it like turning down the volume on a pain signal.

RFA may be used to treat pain from:
✔️ Arthritis in the neck or back (facet joints)
✔️ Chronic shoulder pain
✔️ Chronic knee pain—including some patients who continue to have pain after a total knee replacement
✔️ Certain types of hip pain

The goal of RFA is to help reduce pain and improve function so you can get back to the activities you enjoy.

Not everyone is a candidate for RFA.

Before recommending this treatment, we perform a thorough evaluation to determine whether the painful joint is truly the source of your symptoms. For many spine conditions, this includes diagnostic nerve blocks before considering radiofrequency ablation.

At Interventional Spine & Pain Institute, we'll work with you to determine whether RFA—or another treatment—is the best option for your specific condition.

🩻 MRI Monday: What Are Enlarged Facet Joints? (Facet Hypertrophy)

Have you ever read your MRI report and seen the words "facet hypertrophy"?

It sounds complicated, but it simply means the small joints in the back of your spine have become enlarged from arthritis.

Here's why:
🦴 Whenever two bones meet, they form a joint.

Just like the joints in your knees, hips, or fingers, the small facet joints in your spine can develop arthritis over time. As your body responds to years of normal wear and tear, it may lay down extra bone in an attempt to stabilize the joint. This is what doctors call facet hypertrophy.

Common symptoms may include:
✔️ Pain when getting out of bed in the morning
✔️ Pain getting in or out of the car
✔️ Pain with twisting or side bending
✔️ Low back or neck stiffness
✔️ Pain that improves a little once you get moving

Here's the important part:
Many people have facet hypertrophy on MRI and never experience pain.

At Interventional Spine & Pain Institute, we don't just treat MRI findings. We combine your symptoms, physical examination, and imaging to determine whether the facet joints are truly the source of your pain.

If they are, there are several treatment options that may help, including physical therapy, medications, diagnostic injections, and for appropriately selected patients, radiofrequency ablation (RFA) to help relieve pain.

🦵 What's Wrong Wednesday: Sciatica vs. Spinal Stenosis

Many people use the terms "sciatica" and "spinal stenosis" interchangeably—but they aren't the same condition.

While both can cause pain that travels into the legs, they often have different patterns and different treatments.

⚡ Sciatica
Sciatica usually occurs when a nerve becomes irritated or compressed.
Common symptoms may include:
✔️ Sharp or shooting pain down one leg
✔️ Burning, tingling, or numbness
✔️ May occur sitting, standing, walking, or lying down
✔️ Often starts after lifting, gardening, travel, sports, or another activity
✔️ May improve over several days or weeks with rest, anti-inflammatory medication, ice, heat, or physical therapy

🚶 Spinal Stenosis
Spinal stenosis occurs when the spaces around the spinal nerves become narrowed.
Common symptoms may include:
✔️ Legs feel heavy, tired, or weak when standing or walking
✔️ Pain often improves with sitting or leaning forward (like leaning on a shopping cart)
✔️ Symptoms usually return when standing or walking again
✔️ Often develops gradually over time

Although these conditions can seem similar, the treatment may be very different.

The good news is that many treatment options are available, including physical therapy, medications, injections, minimally invasive procedures, and, in some cases, surgery.

At Interventional Spine & Pain Institute, our goal is to determine the true source of your symptoms so we can recommend the treatment that's right for you.

MRI Monday: What Are Osteophytes?

Have you ever read your MRI report and seen the word "osteophytes"?
Don't worry—you probably know them by another name:
Bone spurs.

Bone spurs are extra pieces of bone that your body may form over time as part of the natural aging process or in response to arthritis.

Think of it like this:
🌳 A tree grows thicker bark as it gets older.

Your spine sometimes responds in a similar way by laying down extra bone around joints that have experienced years of wear and tear.

Here's the good news: Most bone spurs never cause symptoms.

However, if a bone spur grows large enough to press on a nearby nerve or narrow the space where the nerves travel, it can sometimes contribute to:
✔️ Neck or back pain
✔️ Pain traveling into an arm or leg
✔️ Numbness or tingling
✔️ Weakness in some cases

The important thing to remember is that seeing a bone spur on your MRI does not automatically mean it's the source of your pain. Many people have osteophytes and live active, pain-free lives.

At Interventional Spine & Pain Institute, we combine your MRI findings with your symptoms and physical examination to determine whether a bone spur is actually contributing to your condition and discuss the most appropriate treatment options.

💉 Therapy Thursday: What Is an Epidural Steroid Injection?

One of the most common questions we hear is:
"Is this the same epidural women get during childbirth?"

Not exactly.

Both procedures involve the epidural space, but they are performed for different reasons.👶 A labor epidural is designed to provide numbness for childbirth.

💉 An epidural steroid injection is designed to reduce inflammation around irritated spinal nerves to help relieve pain—it is not intended to make you numb or unable to move.

These injections may be recommended for patients experiencing:
✔️ Neck pain that travels into the arm
✔️ Back pain that travels into the leg (sciatica)
✔️ Pain caused by a herniated disc or spinal stenosis
✔️ Persistent pain after spine surgery (in some patients)

The goal is to reduce inflammation, improve comfort, and help you return to your normal daily activities. Every patient is different, and not everyone is a candidate for this procedure.

At Interventional Spine & Pain Institute, we'll review your symptoms, physical examination, and imaging to determine whether an epidural steroid injection—or another treatment—is the best option for you.

Epidural steroid injections are not appropriate for every patient or every cause of pain. The decision to perform an injection depends on your medical history, physical examination, imaging studies, and overall clinical condition. If you're experiencing persistent neck pain, back pain, or pain traveling into your arms or legs, please consult a qualified healthcare provider to determine the most appropriate treatment for you.

🩻 MRI Monday: What Is Disc Desiccation?

Have you ever looked at your MRI report and wondered what "disc desiccation" means?

Despite the complicated name, it's actually a very common finding.
Disc desiccation means the disc has lost some of its water content over time.
Think of it like a grape slowly becoming a raisin. 🍇➡️🍇

Healthy spinal discs contain a lot of water, which helps them act as cushions between the bones of your spine. As we age, those discs naturally lose water, becoming a little thinner and less flexible.

Here's the important part:
Many adults have disc desiccation on MRI and never experience pain.

An MRI is only one piece of the puzzle. At Interventional Spine & Pain Institute, we don't just look at the images—we listen to your symptoms, perform a thorough physical examination, and determine whether the MRI findings actually explain your pain.

If you've been dealing with persistent neck pain, back pain, or pain that travels into your arms or legs, our team can help determine the source of your symptoms and discuss the treatment options that may be right for you.
