Happy Community Health Worker (CHW) Appreciation Week! recognizes the contributions CHWs make in providing care to members across the state. CHWs help improve the health of members with services such as referrals to social service programs, assistance with redetermination applications, and help finding a primary care provider.

This CHW Appreciation Week, we’re highlighting Anna D’Onofrio. Anna has worked at CHNCT since 2013, serving local communities for more than 22 years. She has provided countless members with the support and assistance they need to thrive. According to Anna, the most rewarding thing about this work is the opportunity to make a positive impact on people’s lives. “I am passionate about empowering individuals to overcome barriers, improve their well-being, and achieve their personal health goals. Bringing hope, encouragement, and practical support to those in need is what motivates me every day!”
