For more than four decades, rapamycin has functioned primarily as a daily transplant medication. However, expanding translational research and human trials are now evaluating how low-dose, intermittent regimens impact the biology of aging.

In aging tissues, the mechanistic target of rapamycin (mTOR) often remains persistently activated, which suppresses autophagy and permits damaged cellular components to accumulate. Low-dose rapamycin binds FKBP12 to inhibit mTORC1, releasing the biological brake on cellular cleanup.

Why does longevity dosing differ from traditional protocols?

• Intermittent versus continuous exposure: Transplant protocols rely on daily dosing, which eventually suppresses mTORC2 and drives unwanted metabolic shifts. Longevity protocols utilize a once-weekly pulse to inhibit mTORC1 followed by a multi-day recovery window.
• Dosing parameters: Clinicians typically target three to 10 mg once weekly (commonly titrating to six mg weekly over three to four weeks), with clinical reassessments at 60 to 90 days.
• Formulation considerations: Commercial sirolimus is available only in 0.5 mg, one mg, and two mg tablets, making weekly titration cumbersome. Furthermore, because rapamycin is acid-labile and degrades at gastric pH, precise capsule compounding is necessary to ensure consistent bioavailability.

Recent clinical literature, including the 48-week PEARL trial published in Aging, indicates measurable changes in lean tissue preservation and inflammatory markers under monitored weekly protocols.

Practitioners can review baseline monitoring recommendations, contraindications, and dosing equivalencies through WeCare Pharmacy.

When clinical stress or acute viral exposure challenges the immune system, three foundational micronutrients are depleted before others: glutathione, ascorbic acid, and zinc.

Oral supplementation faces well-documented gastrointestinal absorption thresholds. Intestinal vitamin C transporters saturate near 200 to 400 mg per dose, while oral glutathione undergoes substantial degradation in the digestive tract before reaching systemic circulation. Intramuscular (IM) injection bypasses these intestinal rate limits entirely.

The Tri-Immune formulation combines:

• Glutathione (200 mg/mL): The primary intracellular antioxidant that neutralizes reactive species and preserves leukocyte viability.
• Ascorbic Acid (200 mg/mL): Concentrates in neutrophils and lymphocytes at 50 to 100 times plasma levels, fueling phagocytic activity.
• Zinc Sulfate (five mg/mL): A critical cofactor for thymulin, the zinc-dependent thymic hormone governing functional T-cell maturation.

These ingredients work in synergy. Ascorbate continuously reduces oxidized glutathione back to its biologically active state, while glutathione recycles dehydroascorbate. Administering either nutrient in isolation when a patient is deficient in both accelerates depletion.

Clinical applications include:

• Acute onset protocols (one mL IM daily for up to three consecutive days)
• Proactive defense before travel, medical procedures, or high-exposure periods
• Maintenance protocols for patients prone to lingering upper respiratory infections (URIs)
• Fast in-office support on non-infusion weeks for IV nutrient patients

Tri-Immune is compounded by WeCare Pharmacy under stringent sterile preparation standards and supplied in multidose vials for in-office administration.
