The Hawaiʻi State Health Planning and Development Agency (SHPDA), Department of Health — Ka ʻOihana Olakino, recently shared an important announcement requesting feedback from physicians and clinicians regarding the new CMS Rural Health Transformation Program (RHTP).

This program may impact independent practices, rural-serving providers, primary care, specialists, EHR readiness, care coordination, value-based care preparation, and future ACO/APCM opportunities.

What is being considered?

SHPDA is exploring a working concept that may include a statewide Clinically Integrated Network (CIN). A CIN is intended to help providers work together on care coordination, quality improvement, data sharing, technology integration, and value-based care goals while allowing practices to remain independent.

The proposed structure may also support practices with:

• EHR upgrades, training, interfaces, and connectivity
• Practice transformation support
• Care coordination staffing support
• Technical assistance for value-based care preparation
• AHEAD, ACO/MSSP, MIPS, and APCM-related pathways
• Possible incentive opportunities for participation and reporting activities

Why this matters

This could create new opportunities for Hawaiʻi providers to receive support for the administrative, technology, and care coordination work needed to prepare for future value-based care models.

Provider feedback requested

SHPDA is asking providers to respond to these key questions:

Would you be interested and would you participate in this structure?
Are the required activities reasonable?
Does the opportunity overcome the potential administrative burden?
Are there any requirements that would prevent independent practices from joining?
What would need to change for this to work in the real world?

Provider Reminder: Metabolic Monitoring Matters

Patients with severe mental illness (SMI) and those prescribed antipsychotic medications may face increased risks for metabolic syndrome, diabetes, and cardiovascular disease.

Routine metabolic monitoring is a simple but important step that supports early identification, timely intervention, and better long-term patient outcomes.

For providers and billing teams, this is also a reminder to make sure these services are:

✅ Ordered when clinically appropriate✅ Completed according to monitoring guidelines✅ Documented clearly in the medical record✅ Billed correctly based on payer policy and covered services✅ Coordinated between behavioral health, primary care, and lab services when needed

When care is properly monitored, documented, and billed, practices can support quality patient care while also protecting appropriate reimbursement.

Hawai‘i CCS Contract Update

The Community Care Services (CCS) transition timeline has been updated.

AlohaCare was awarded the statewide CCS contract, but the transition start date is now expected to begin October 1, 2026 instead of the previously expected July 1, 2026 date.

Providers should continue monitoring official Med-QUEST, AlohaCare, and current CCS plan communications for transition guidance, authorization updates, billing instructions, and network notices.

BAC-Office will continue watching for updates that may impact billing, credentialing, and provider workflow preparation.
