Ask ten women what perimenopause looks like and you'll get the same three answers. Hot flashes. Trouble sleeping. Weight gain.

That's the picture almost everyone carries around, including a lot of clinicians.

Now here's what women actually report.

Data published this year in the journal Menopause. 17,494 women across 158 countries.

Most commonly recognized as perimenopause: Hot flashes 71% Sleep problems 68% Weight gain 65%

Most commonly reported by women 35 and over: Fatigue 83% Exhaustion 83% Irritability 80% Depressive mood 77% Digestive symptoms 76% Anxiety 75%

Stack those two lists against each other and you've got the whole problem in one picture.

The most common symptoms of this transition are the ones almost nobody associates with it. So you walk into an appointment carrying the real symptoms, you never say the magic word, and neither does the person across the desk from you.

And it doesn't get called perimenopause. It gets called depression. Anxiety. Stress. "You're a busy mom."

You are not imagining this and you are not describing it badly. You're describing it accurately, to a picture that was never drawn right.

Progesterone is supposed to make you feel calmer. But what happens when it doesn’t?

For some women, changing hormone and neurosteroid signaling may contribute to a very different experience—anxiety, agitation, feeling wired, or simply not feeling like yourself.

That doesn’t automatically mean progesterone is wrong for you.

Dose, timing, formulation and your individual hormonal picture matter.

If you started hormones expecting to feel better—but felt worse—your response deserves an explanation.

Send this to a woman who needs to hear it. Let us know if this was you!

Ferritin of 14. Four doctors. Four "normal" labs.

She was 46, autoimmune thyroid disease, vegan for ten years — and she sat in my office apologizing for crying.

Here's what nobody told her: autoimmune disease raises the bill. Your immune system runs hot around the clock, burning through raw materials, and almost nobody adjusts what's coming in to match. Menopause doesn't end that. The burn doesn't stop.

And you don't build blood from iron. Step one is glycine, an amino acid concentrated in animal protein. Iron gets added at the end — which is why iron alone so often does nothing.

Ten years of a diet that's fine for most people. She wasn't most people.

Ask what your ferritin number was. Not whether it was "normal."

Drop your ferritin number in the comments. I want to see how many of you are sitting under 30 and were told you were fine.

They called it anxiety at forty-two. Nobody once said the word perimenopause.

And you've never been an anxious person — anyone who knows you would say that. Now your heart is pounding at three in the morning, you walked out with a prescription in under ten minutes, and nobody asked why it started this year.

Anxiety is real. Sometimes medication is exactly the right answer, and I'm not arguing that. I'm asking why now.

Here's how it works. Progesterone is your calming hormone. Your body converts it into something called allopregnanolone, and that's the piece doing the actual calming — it pushes on the same brake pedal in your brain that anti-anxiety medication pushes on. In perimenopause, progesterone drops first, and it drops hard. Same life. Same stress. Weaker brake. And now it feels like panic.

That's not you becoming an anxious person. That's you losing your brake.

Here's how you find out for free, starting tonight. Every time it hits, write down the date. Just the date. Do that for two cycles, then look at where those dates land. If they're stacking up in the week or two before your period, that isn't a coincidence.

Now you're not walking in saying "I feel anxious." You're walking in with a pattern.

At your next appointment, say this: "Did my anxiety start, or change, at the same time as my sleep or my cycle?"

Send this to whoever got handed a prescription and never got asked why now.

Hysterectomy but kept your ovaries? They said six weeks. It's been two years.

The bleeding stopped. Nothing else did. You're exhausted, you're not sleeping, and you feel ten years older than the day you walked in. You've stopped bringing it up, because the thing they said would fix it is already gone.

Here's what nobody explained before you signed. Your ovaries have two blood supplies — one down from the aorta, one across from the uterus. Take the uterus out and one of those roads goes with it. Your ovaries are still there. They're just running on less, and in a lot of women the uterine side was carrying most of the load.

Duke followed over eight hundred women. Those who kept both ovaries were roughly twice as likely to have them fail — menopause three to four years early. How much is the surgery versus the condition that put you in the operating room isn't settled. The risk shows up either way.

And the one signal that would have warned you — your cycle changing — was surgically removed. So somebody has to go look on purpose.

At your next appointment, say this: "My ovaries were left in. Has anyone actually checked whether they're still working?"

HRT not working perimenopause menopause — if hormone therapy helped some of it and not all of it, that is not a failure and it is not in your head. A group of gut bacteria called the estrobolome metabolizes your estrogen and determines how much gets used versus recycled - which is part of why two women on the same dose can have completely different results. It isn't on a standard hormone panel.

Tomorrow, 7pm EST, live and free: how your gut, your hormones, and your metabolism are actually connected, the tests that find root causes when standard labs come back "normal," and what to do about it. 45 minutes.
