Zufall Health's Community Health and Advocacy Team (Z-CHAT) is here to provide linkage to care for People Living With HIV (PLWH) and those at risk. We can help with medical, dental and behavioral health care; STI/STD testing; health insurance enrollment; food, housing and legal assistance; and other services for PLWH including utility and rental assistance. All treatment and testing is handled wit

h the highest level of care and confidentiality. Our board-certified physicians, case managers, and community health workers care for patients regardless of income and insurance status. A sliding free discount is available to ensure affordable access to all services. Additionally, we provide translation services and transportation assistance to your appointments.
