If you are over 45 and filler is not giving you the result it used to, there is usually a reason for that.

✨Endoscopic Deep Plane Facelift Is Not A Mini Lift ✨

One of the biggest misconceptions I hear about an Endoscopic Deep Plane Facelift is that hidden incisions must mean it’s a Mini Facelift.

I understand why patients think that. When people hear “hidden incisions,” they often assume it is just a mini version of a facelift. But that is not necessarily true with the endoscopic deep plane approach that I perform.

The real question is not just where the incisions are. It is whether those incisions can stay hidden while still allowing me to do a strong, deep-plane lift. In the right patient, the answer is yes.

With this approach, the incisions are designed to stay as hidden as possible — in the temporal hairline, under the temporal tuft, and around the earlobe and behind the ear. But the goal is not just smaller scars. The goal is to give me access to release the deeper tissues, lift them vertically, and secure them to stable structures like the deep temporalis fascia and mastoid periosteum.

That is why this is not just surface tightening. It is still a true deep plane facelift in the right candidate.

At the same time, this approach is not for everyone. If a patient has a lot of soft-tissue descent and excess loose skin, then hidden incisions alone may not be enough, and an open approach may be the better choice.

The right way to think about it is this: hidden incisions do not automatically mean a mini lift. In the right patient, they can still support a strong, natural-looking lift without giving up the deeper work.

✨ Why a Neck Lift Alone Would Not Have Created This Transformation ✨

When we see looseness under the chin and heaviness in the neck, it’s usually not isolated to that area. Sagging is not just a loose skin problem.

I educate my patients all the time that the face and neck age together. The neck is rarely aging alone. As the deeper facial ligaments weaken, the fat pads and muscle descend as one unit, and the sagging in the face pushes into the neck.

🧠 Jowls and neck laxity are caused by deeper soft tissue descent. It’s not just loose skin. The fat pads lose structural support and shift downward. The platysma muscle and neck tissues follow. A necklift alone cannot improve contour and address all the loose skin. When we understand what is falling, we can treat it appropriately.

That’s why when it comes to sagging and loose skin, a necklift is combined with a facelift. A facelift repositions the deeper support structures of the face. A necklift refines and contours the neck. Doing both together creates a smoother, more natural transition from the cheeks to the jawline to the neck.

✨ The Biggest Myth About Endoscopic Facelift ✨

One of the biggest myths I hear about an Endoscopic Facelift is that it must wear off faster than an open facelift just because the incision is smaller.

I understand why patients think that. A smaller incision sounds like it must mean a mini lift or a less durable result. But that is not actually the best way to think about longevity.

In my hands, both an endoscopic and an open deep plane facelift are lifting the tissues in a similar way and securing them to stable structures like the deep temporalis fascia and the mastoid periosteum. So when patients ask me which one lasts longer, the bigger variable is often not the incision style — it is how the face continues to age after surgery.

Here’s what matters over time: collagen loss, tissue strength, volume loss, bony changes and maintenance. Those are the things that affect when someone starts noticing sagging again. The technique still matters for how natural the result looks and how strong the initial lift is, but long-term aging is often the bigger factor.

So the better question is not just, “How long does an endoscopic facelift last?”
It is: “How will my face age after surgery, and what can I do to maintain my result?”

✨ Deep Plane Facelift at 39 ✨

Why I Combine a Deep Plane Facelift with a Deep Neck Lift and Deep Neck Contouring ✨

Jordan was on a weight loss journey. She wanted to address her heavy neck and the lack of definition, along with her jowls and the laxity in her face. In her case, I recommended a Deep Place Facelift, Deep Neck Lift, Deep Neck Contouring and Rhinoplasty to address her concerns.

When I see a patient with a very heavy neck and laxity in the face face, I know I am not dealing with just one problem.

In these cases, the face has descended, but the neck also has its own deeper heaviness. That is why I often recommend combining a Deep Plane Facelift with a Deep Neck Lift and Contouring rather than trying to treat only one area.

A deep plane facelift helps me reposition the tissues that have fallen through the cheeks and jawline. That is what improves jowling, lower face heaviness, and loss of definition along the jawline. But when the neck is very full or structurally heavy, that part usually needs to be treated directly as well.

A deep neck lift and deep neck contouring allows me to address the deeper layers of the neck. That may include deeper fat, platysma laxity, and excess skin - Liposuction alone will not address these. This is why the combination is so important in the right patient. It allows me to treat the face and neck as one unit so the result looks more balanced, more natural, and more complete.

The simplest way I explain it is this:
When the jawline is blurred and the neck is both full and loose, I need to lift the face where it has fallen and sculpt the neck where it is heavy.

✨ When Will I Look Presentable After a Facelift? ✨

Jennifer is a great example of why facelift recovery happens in phases, not all at once. She had an endoscopic deep plane facelift, deep neck lift, deep neck contouring, facial fat transfer, and rhinoplasty, her healing was going to be a process of gradual changes rather than one sudden transformation.

🗓 Week 1: You’re swollen and bruised, but you expected it — this is pure healing mode. Rest and stay patient.

🗓 Week 2: Swelling begins to come down, but you may notice unevenness. That’s normal as each side heals at a different pace.

🗓 Week 3: This is often when patients start to feel discouraged. Visible improvement slows down, but deeper tissues are still healing.

🗓 Week 4: You start to recognize yourself again. Most patients feel comfortable seeing close friends or going out for a quiet dinner.

🗓 Weeks 5–6: The swelling continues to fade, your expressions start to look natural again, and you begin to see yourself more - the “refreshed you.”

While you’ll look better and feel more confident usually by 6 to 8 weeks, full recovery takes up to one year for every layer to completely settle.

If you had a combination of procedures, expect your healing to take more time because those can add additional swelling and recovery time.

If you’ve ever had fillers (even after dissolving), certain skin tightening or nonsurgical treatments, these could also contribute to more swelling and prolong your healing.

Recovery takes time. There is no magic wand. It’s about letting your body do what it does best to heal. More importantly, give yourself time. Each phase reveals more of your final, natural result.

✨ Endoscopic Deep Plane Facelift - Where Does the Skin Go? ✨

One of the most common questions I hear is: “If you’re not cutting skin, where does the extra skin go?” It’s a great question - and the answer comes down to how the lift is done and who is a good candidate. In an endoscopic facelift, I’m still performing a deep plane facelift. The lift happens in the deeper layers of the face, not by pulling on the skin itself. Because the deeper structures are lifted back to where they used to be, the skin is carried along naturally. In the right patient, little to no skin needs to be removed - and the skin has the ability to recoil and redistribute smoothly on its own.

🧠 This fully Endoscopic Deep Plance Facelift approach works best in patients with good skin quality and elasticity, and who do not have a lot of laxity. When skin has strong “bounce back,” it can adapt to the lift without needing large skin excisions. Even in traditional open deep plane facelifts, the amount of skin removed is often just a few millimeters - far less than most people expect. That’s why candidacy matters. An endoscopic facelift isn’t about skipping steps — it’s about using the right technique for the right anatomy to achieve a natural, balanced result.
