Something most people don't think about when they're signing up for Medicare is cancer insurance.

Medicare covers a lot when it comes to treatment, but there are costs that tend to slip through the cracks. Travel to a treatment center, time away from work during recovery, meals, lodging, things that are really hard to plan for ahead of time.

A cancer plan pays you a lump sum if you're ever diagnosed, and you can use that money however you need it. It's not complicated and it's pretty affordable as far as add-ons go.

Hey. One of the most common things I hear from people new to Medicare is "I thought Medicare covered everything."

It doesn't. And the gaps can catch people off guard pretty quickly.

Here are a few of the big ones worth knowing about before you make any coverage decisions.

The Part B 20% β€” After your deductible, Original Medicare pays 80% of most doctor and outpatient costs. You're responsible for the other 20%. What most people don't realize is there's no cap on that. A $100,000 surgery means you're looking at a $20,000 bill out of pocket.

The Part A hospital deductible β€” In 2026 that's $1,736 per benefit period. Not per year. Per benefit period. So if you're hospitalized more than once, you could pay it more than once.

Skilled nursing facility costs β€” Medicare covers the first 20 days of a skilled nursing stay at 100%. Starting day 21, you're paying $217 a day out of pocket. That adds up fast.

Dental, vision, and hearing β€” Original Medicare doesn't cover routine dental work, eye exams, or hearing aids. Most people are pretty surprised by that one.

None of this is meant to scare you. It's just useful to know what you're actually working with before you decide how you want to be covered.

Here's something a lot of people find out too late.

You only have 7 months to sign up for Medicare when you turn 65. That window opens 3 months before your birthday month and closes 3 months after.

Miss it, and the federal government charges you a late enrollment penalty that sticks with you for life.

Now, there are exceptions. If you're still working at 65 and covered under an employer health plan through a current employer (with 20 or more employees), you may be able to delay Medicare without any penalty. You'd get a Special Enrollment Period when that coverage ends.

But here's the catch: a lot of people assume they qualify for an exception when they don't. Retiree coverage, COBRA, and marketplace plans do NOT protect you from the penalty.

I saw this confusion all the time as an RN. It's a big reason I became a broker.
