TESTOSTERONE CREAMS IN WOMEN: WHY DHT MATTERS

Topical testosterone can work very well for women—but there’s one issue that deserves more attention:

DHT conversion.

When testosterone is applied to the skin, it encounters 5-alpha-reductase (higher amounts in skin), an enzyme that converts testosterone into the much more potent androgen DHT (dihydrotestosterone).

Transdermal testosterone can produce proportionally higher DHT exposure than injectable testosterone.

For women, this matters because excessive androgen/DHT activity may contribute to:

• Scalp hair thinning or shedding
• Acne and oily skin
• Increased facial/body hair
• Other unwanted androgenic effects

Women require very small testosterone doses, so the formulation, application site and absorption can make a significant difference.

This doesn’t mean testosterone cream is “bad.”
Many women do very well with it.

But if a woman on topical testosterone develops hair loss, acne or unwanted hair growth, don’t automatically assume testosterone itself is the problem.

Look at DHT and consider the delivery method.

Sometimes changing the route of testosterone administration can make just as much sense as changing the dose.

Same hormone. Different delivery. Different metabolism.

This is primarily why I personally recommend injections over topicals for women specifically.

The route matters.

Total gaslighting nonsense.

Plenty of options are out there for properly dosed testosterone for women.

The vast majority of primary care and even gynecologists won’t support it simply because they were never taught.

Aspire Rejuvenation Clinic has been doing this for nearly a decade. Testosterone is off label use. Just like Aspirin is for 100 things and Metformin for PCOS among thousands of other drugs.

There are special formulations for women, doses, and clear protocols.

Women are making noise (as they should) and it’s just now being heard!

NO APPROVAL: Women seeking testosterone for menopause face insurance denials, dosing guesswork, and zero FDA-approved options.

U.S. health regulators met Thursday to evaluate whether standardized, safer access to testosterone therapy for women can move forward.

Doctors currently prescribe micro-doses of male products or compounded formulations, leaving patients to measure fractions of gel packets at home with syringes — and sometimes seeing no results after months of effort.

It's the "buzz word" in perimenopause and menopause and on social media.

Weight gain
Poor sleep
Cravings
Anxiety

It HAS to be cortisol right??!!
Well it likely is but checking a level is rarely fruitful.

Patient: "I have all the cortisol symptoms but my levels are normal."

It's not so much the cortisol level but the counter-regulatory measures your body loses in menopause that make you become symptomatic.

We'll discuss it more in today's free post. https://substack.drjoeclark.com

Your hair is something that may be taken for granted.....until it's a problem. Hair loss can be the "warning light" on the dashboard saying there is a bigger, more concerning problem at hand.

How do you systematically approach this problem?
What are some causes?
What are the solutions!?

Most importantly.....how long will it take???

"The first diagnostic test in every consultation is the conversation."

- Dr. Clark

I am always humbled at how speaking with a patient invites you into their lives, their struggles, their pain and their search for answers. I treat this with the utmost respect. Being "heard" can sometimes be the BEST medicine.
