🚫 No insurance? You can still get personalized, relationship-centered primary care at Community Roots.

🌿 After multiple inquiries, we have developed a self-pay structure designed specifically for adult patients 18+ who do not have health insurance coverage, but are still looking to secure primary care services at Community Roots Integrative Health and Primary Care.

• Comprehensive new patient visit — $250 (60 minutes)
• Extended office visit — $175 (40 minutes; multiple concerns)
• Routine office visit — $150 (25 minutes; one concern)
• Acute or sick visits — $125

Self-pay patients will need to complete a new patient comprehensive visit to establish care with Community Roots before booking extended, routine, or acute/sick visits, as we are not an urgent care facility for non-community roots patients. Acute and sick visits are for established primary care patients with Community Roots only.

Prices listed cover the appointment. Testing and/or procedures will incur additional costs, where applicable. Payment is due at the time of your visit. Concierge membership can additionally be added for interested patients who want a more personalized approach to their healthcare at Community Roots.

🌿 The Concierge Primary Care Membership 🌿

An optional enhanced-care experience for patients who want greater access, more time, and more personalized support from their primary care provider. Membership is designed to complement your traditional primary care services by providing additional access, direct communication, enhanced care coordination, and proactive health planning that extends beyond the standard office visit— not rushed appointments and volume-driven medicine.

Unlike traditional primary care practices that may care for thousands of patients, we strive to restore the patient-provider relationship. Seeing the same provider, a provider that knows you and your medical history, consistently.

Concierge primary care intentionally maintain a smaller patient panel. This allows for longer visits, more direct communication, faster scheduling, and a deeper understanding of each patient’s overall health and goals.

We are accepting the first 100 patients interested in a concierge membership for enhanced primary care access at an office-opening introductory rate of $99 a month (or $1100 up front annually). Couples $179 a month (or $1,975 up front annually), or household $249 a month (or $2,750 up front annually). Household membership includes up to 3 adult members 18+ residing in the same household. Additional members $50/month. Rates will increase after the first 100 patients.

✨ As we move closer towards opening our office, patients interested in securing one of the introductory membership rates are encouraged to email us at [email protected].

I have recently earned a Women’s Health and Hormone Certification which included 46 ANCC-accredited continuing education hours, with 21 hours focused on pharmacology. This additional training supports my work in women’s health, PCOS/endometriosis, hormone-related concerns, perimenopause and menopause care, sexual health, and whole-person treatment planning. It allows me to offer more focused, evidence-informed care for women navigating symptoms, transitions, and health concerns that are too often ignored, undermanaged, or overlooked.

🌿 ABOUT ME: Before I am a healthcare provider, I am a person — a mom of three, a partner, a dog mom.

My career in healthcare began in 2010 in emergency medicine, where I spent 12 years as an ER nurse caring for people during some of their most vulnerable moments. In 2022, I became a Family Nurse Practitioner, spending three years in urgent care before transitioning into primary care for a better work-life balance.

Each of those experiences influenced & shaped the provider I am today. Emergency medicine taught me how to remain calm, think critically, & care for the person behind the diagnosis. Urgent care strengthened my ability to evaluate and treat a wide range of concerns. But primary care showed me what I had been missing: the opportunity to truly know my patients.

It also made me increasingly aware of the limitations & downfalls of modern-day healthcare.

Modern medicine has incredible resources, technology, & expertise — but somewhere along the way healthcare became increasingly focused on volume, productivity, financial gains. Appointments became shorter. Providers became busier. And patients are left feeling unheard, rushed, or like just a medical record number or a dollar sign.

To be frank, I learned rather quickly that I couldn’t practice that way.

I’ve always believed that if you want something to change, sometimes you have to be willing to create the change yourself. So I followed my intuition & trusted my heart. Community Roots was born from that belief — a desire to bring healthcare back to what it should be: personal, relational, & centered around the patient. With personal care driven by patients, for patients, not quotas. I wanted to create a practice where there is time to listen. Time to ask questions. Time to understand what is happening in someone’s life that may be affecting their health and their choices.

I have recently also earned a Women’s Health and Hormone Certification which included 46 ANCC-accredited continuing education hours, with 21 hours focused on pharmacology. This additional training supports my work in women’s health, PCOS/endometriosis, hormone-related concerns, perimenopause and menopause care, sexual health, and whole-person treatment planning. It allows me to offer more focused, evidence-informed care for women navigating symptoms, transitions, and health concerns that are too often rushed or overlooked.

With a blend of creativity, empathy, & A LOT of heart, my mission is to restore the foundational patient-provider relationships that have been lost in modern corporate medicine. Personal. Intentional. Collaborative. And rooted in the belief that exceptional care starts with truly knowing the person you’re caring for. Thanks for stopping by!
