Community First Home Health Care, LLC believes in providing comprehensive, quality care that is client-centered and tailored to assist the individual and the family in achieving optimal level of health. To this end, we are tirelessly pursuing continuous improvement in our service through:

• Providing high quality of comprehensive health care services through a coordinated plan of treatment to the

ill, aged, and disabled residents of the community.

• Providing continuity of health care services in the referral mechanism to facilitate uninterrupted care for the patient.

• Providing the psychological and rehabilitative stimuli for early recovery, as well as maximum rehabilitation, of the patient in the home environment.

• Providing appropriate environmental change in situations where such action is indicated, for health promotion and health maintenance of the patient and their family.

• Providing positive channels of communication with the physicians, health and welfare agencies and the residents of the community.

• Providing an environment that is conducive to employee growth, with ample opportunity for academic improvement through comprehensive and continuous inservice learning experience.

Due to the Jones Fire currently affecting Wickenburg, our physical office is temporarily closed for the safety of our staff and community.

Please note that this closure applies to our office location only. Agency operations continue as usual, and our team remains fully available remotely to support our patients and clinicians.
