Hi everyone! I just wanted to touch base on a couple of things so everyone is on the same page. I know there has been a lot of confusion with the clinic transition so in efforts to make this as smooth as possible, here are a few housekeeping items:

1. County Line Health Clinic is now permanently closed and any communication will need to be through High Plains from now forward.

2. I am only working 2 days per week at High Plains in effort to have more time for my family and my health. Unfortunately, this means I will not be able to see everyone on my prior patient panel and my schedule is very limited. I understand that may be frustrating, and I sincerely apologize for any inconvenience or distress this may cause you; however I want to reiterate, there are now a lot of great providers at High Plains and the goal is always to make access to care more readily available for you and to ensure you are taken care of in a timely manner- even if that means seeing another provider.

3. I just started at High Plains on Tuesday 6/23 and am currently in training under their system. It is a lot different than our system so that takes some time.

4. I do not start seeing patients until next week on 6/30 and only a few patients a day- again, while I learn their system. Because of this, as of right now, I personally do not have any same day appointments; however you can call the clinic and see one of the other providers for a sick visit any day of the week. I may have some sick call appointments in the future once we settle in.

5. Out of respect for my personal time, I ask that you please not private message or call my personal phone about work related items outside of work hours. I have had an overwhelming amount of calls, texts, and messages- often about things that are out of my control. If you need me, I’ll be in the office every Tuesday and Wednesday or you can leave a message with support staff at High Plains and they will communicate with me when I am available. Again, I understand that reducing my hours makes me less readily available; however, this is something I needed to do for my family and ask for some grace in this transition as well. That being said, it does mean the world to me that you have such trust in me, and I am thankful for the relationships we have built.

Thank you all, again, for the tremendous support and backing over the past 5 years and as we continue into this transition. Hang in there! It will all work out 😊 Hopefully I will be seeing many of you soon!

Enjoy your Summer,

Shelby

The emotions.

Our last day.

A period at the end of the sentence… a door closed… the end of a chapter... a vision achieved… a job well done.

But also a semicolon- another season ahead of us… another door opening… a time
to plant new seeds… and a time to pluck out what’s been planted…

Time to plow it up, rework the ground, and plant seeds for a new harvest. A new season.

A sigh of relief after years of hard work, but a heaviness for what is left behind. The emotions.

Thank you all for going on this journey with us! Your trust and support have meant more than I could ever express! Here’s to new seasons and new growth!

Take care, be well!

If you need me, I’ll be at the ballfields 🙌🏻⚾️

You’re hometown provider,

🤍Shelby

‼️IMPORTANT ANNOUNCEMENT‼️

⚠️There have been a lot of inaccurate rumors circulating so I wanted to address this publicly so you hear it straight from my mouth. I AM NOT LEAVING/QUITTING. I want to reiterate that first! ⚠️

To my faithful and supportive patients, family, and friends,

It is with a heavy heart that I make this announcement. I want to first say that I have not made this decision lightly, and I have spent months in careful and prayerful consideration. Due to personal health issues and a desire to spend more time with my children, my husband and I have decided to sell the clinic. Rest assured, we will not be leaving you stranded. High Plains Community Health Center has acquired our practice and will be re-opening a Wiley branch in the near future, following some renovations. In every effort to make this as smooth and seamless of a transition as possible, I plan to stay on with them part time and continue working as part of their provider team. After a short period of time in Lamar, we will eventually transition back to their clinic on Main Street in Wiley. (If you are a patient, you will be receiving a letter with more information.)

It has been a true honor to care for all of you at County Line Health Clinic, and I thank you so much for your trust and support over the past 4-5 years as we took on this lifelong dream! We are proud of what we have built, and we hope you are too! My staff and I have worked hard to ensure all of you have been heard and cared for to the full extent of our abilities, and we hope you have felt that. My prayer is that the County Line Health Clinic legacy will live on in each of you for years to come! Again, it has been a true honor- thank you, from the bottom of my heart, for being such a crucial part of this vision! Stay happy and healthy my friends! See you soon!

Your Trusted Provider,

Shelby Casper, FNP-C
