1. Open the witness slip link https://ilga.gov/House/hearings/details/3051/23078/CreateWitnessSlip/?legislationId=168137&GaId=18&View=Create
2. Fill in your Contact Information
3. Fill in your pharmacy or "representing yourself" in the Representation field.
4. Under Position, Select Original Bill, and Select “PROPONENT”
5. Click Add Position
6. Mark “Record of Appearance Only” unless otherwise instructed
7. Submit your slip
